To provide that each insurer, health care center, fraternal benefit society, hospital service corporation, medical service corporation or other entity delivering, issuing for delivery, renewing, amending or continuing a health insurance policy in this state shall not provide any document to an insured or prospective insured unless such document is in plain language, provide for transparency in the cost of covered health care services and the calculation of cost-sharing payments for such services, not deliver, issue for delivery, renew, amend or continue high deductible health plans in this state and not charge any coinsurance to an insured in this state.
