History An assignment of benefits is a document signed by a policyholder that allows a third party, the assignee, to receive payments for services directly from the policyholder's insurance company. Pursuant to A.R.S. § 20-464, if an insured person assigns payment for benefits under the health care insurance contract to a heath care provider that performed the services covered by the contract the insurer may provide payment directly to the provider. Statute prohibits payment for services to a person other than the one to whom payment has been assigned. Provisions 1.
