AN ACT CONCERNING TH E DEVELOPMENT OF A F ORM TO BE 
USED BY PARENTS WHO OBJECT TO IMMUNIZATION ON 
RELIGIOUS GROUNDS AN D NOTIFICATION TO SCHOOL NURSES 
CONCERNING DISCRETIO N IN ACKNOWLEDGING S UCH 
OBJECTIONS.  
Be it enacted by the Senate and House of Representatives in General 
Assembly convened: 
 
Section 1. (Effective July 1, 2019) Not later than October 1, 2019, the 1 
Department of Public Health shall develop and make available a form 2 
to serve as the statement to be (1) presented, in accordance with the 3 
provisions of section 10-204a of the general statutes, by the parents or 4 
guardians of a child that an immunization required pursuant to said 5 
section is contrary to the religious beliefs of such child or the parents 6 
or guardians of such child, and (2) acknowledged, in accordance with 7 
the provisions of sections 1-32, 1-34 and 1-35 of the general statutes, by 8 
(A) a judge of a court of record or a family support magistrate, (B) a 9 
clerk or deputy clerk of a court having a seal, (C) a town clerk, (D) a 10 
notary public, (E) a justice of the peace, (F) an attorney admitted to the 11 
bar of this state, or (G) notwithstanding any provision of chapter 6 of 12 
the general statutes, a school nurse. The department shall include a 13 
notice on such form that any person authorized to acknowledge such 14 
statement pursuant to section 10-204a of the general statutes has the 15 
discretion to acknowledge or refuse to acknowledge such statement. 16

Sec. 2. (Effective July 1, 2019) Not later than September 1, 2019, the 19 
Department of Education shall send a written notification to each local 20 
and regional board of education and the Association of School Nurses 21 
of Connecticut that states that any school nurse asked to acknowledge 22 
the statement of a parent or guardian of a child that immunization is 23 
contrary to the religious beliefs of such child or the parent or guardian 24 
of such child, pursuant to section 10-204a of the general statutes, has 25 
the discretion to acknowledge or refuse to acknowledge such 26 
statement. Each local or regional board of education shall distribute 27 
such written notification to each administrator, as defined in section 28 
10-144e of the general statutes, employed by such board. 29 
This act shall take effect as follows and shall amend the following 
sections:
