AN ACT REQUIRING THE USE OF THE STATE BOARD OF
EDUCATION'S HEALTH ASSESSMENT RECORD FORM.
Be it enacted by the Senate and House of Representatives in General
Assembly convened:
That section 10-206 of the general statutes be amended to require the 1
form supplied by the State Board of Education be the only acceptable 2
form for health assessment records for the purpose of school enrollment. 3
Statement of Purpose:
To require the form supplied by the State Board of Education be the only
acceptable form for health assessment records for the purpose of school
enrollment.
