AN ACT CONCERNING THE PATIENT -CENTERED MEDICAL HOME -
PLUS PROGRAM. 
Be it enacted by the Senate and House of Representatives in General 
Assembly convened: 
 
Section 1. (NEW) (Effective from passage) (a) As used in this section, (1) 1 
"federally qualified health center" has the same meaning as provided in 2 
42 USC 1395x(aa)(4), as amended from time to time, and (2) "Patient-3 
Centered Medical Home-Plus program" means a person-centered 4 
medical program that provides comprehensive and coordinated care to 5 
HUSKY Health program members under a shared savings model that 6 
allows health care providers who provide quality care at reduced costs 7 
to receive half of any savings. 8 
(b) The Commissioner of Social Services shall pay to a federally 9 
qualified health center participating in the Patient-Centered Medical 10 
Home-Plus program an amount equal to the amount such center was 11 
due under the program in November 2024 and December 2024. The 12 
commissioner shall not make shared savings payments under the 13 
Patient-Centered Medical Home-Plus program until such time as the 14 
arrearage of such payments to a federally qualified health center is 15

rectified. 16 
This act shall take effect as follows and shall amend the following 
sections:

Statement of Purpose:   
To require the Department of Social Services to provide overdue 
incentive payments to federally qualified health centers under the 
Patient-Centered Medical Home-Plus program.
