LCO 1 of 2 General Assembly Substitute Bill No. 1483 January Session, 2025 AN ACT CONCERNING THE PATIENT -CENTERED MEDICAL HOME - PLUS PROGRAM. Be it enacted by the Senate and House of Representatives in General Assembly convened: Section 1. (NEW) (Effective from passage) (a) As used in this section, (1) 1 "federally qualified health center" has the same meaning as provided in 2 42 USC 1395x(aa)(4), as amended from time to time, and (2) "Patient-3 Centered Medical Home-Plus program" means a person-centered 4 medical program, administered by the Department of Social Services, 5 that provides comprehensive and coordinated care to HUSKY Health 6 program members under a shared savings model that provides half of 7 savings achieved to health care providers who provide quality care at 8 reduced costs. 9 (b) The Commissioner of Social Services shall pay to a federally 10 qualified health center participating in the Patient-Centered Medical 11 Home-Plus program an amount equal to the amount such center was 12 due under the program in November 2024 and December 2024. The 13 commissioner shall not make shared savings payments under the 14 Patient-Centered Medical Home-Plus program until such time as the 15 arrearage of such payments to a federally qualified health center is 16 rectified. 17 Substitute Bill No. 1483 LCO 2 of 2 This act shall take effect as follows and shall amend the following sections: Section 1 from passage New section Statement of Legislative Commissioners: Subsec.
