HLS 12RS-1016 ORIGINAL Page 1 of 4 Regular Session, 2012 HOUSE RESOLUTION NO. 140 BY REPRESENTATIVE SIMON HEALTH CARE: Extends the Study Group on Chronic Care Management in the Community A RESOLUTION1 To extend the work of the Study Group on Chronic Care Management in the Community,2 herein referred to as the "study group", to study and make recommendations for3 improving the coordination of care and support services so that persons with chronic4 diseases can remain in their homes and communities as long as possible.5 WHEREAS, by House Resolution No. 125 of the 2011 Regular Session of the6 Legislature, the House of Representatives created the Study Group on Chronic Care7 Management in the Community, thereby recognizing the need to identify solutions for8 improving the delivery of chronic care in the community; and9 WHEREAS, the House of Representatives also recognized the need to improve10 Louisiana's chronic care delivery in the areas of housing, transportation, support services,11 and technology; and12 WHEREAS, the House of Representatives has received an informative preliminary13 report from the study group which addressed key recommendations and strategies for14 improving the coordination of care and support services for persons with chronic diseases;15 and16 WHEREAS, the report included eight key recommendations and strategies based on17 the need to coordinate care differently when addressing people with chronic diseases, which18 include the following:19 (1)Ensuring seamless transitions between care settings.20 HLS 12RS-1016 ORIGINAL HR NO. 140 Page 2 of 4 (2)Improving access to coordination of care, including both health- and1 community-based services.2 (3)Utilizing current and emerging technology to improve care, outcomes, and3 cost. 4 (4)Accessing and utilizing health information on chronic disease for effective5 decisionmaking.6 (5)Supporting family caregivers and engaging them as partners with7 professionals.8 (6)Incentivizing providers, payers, and consumers to improve quality care and9 outcomes for individuals with chronic disease.10 (7)Mobilizing community resources to meet needs of persons with chronic11 disease.12 (8)Creating a public and private partnership that improves the outcomes for13 Louisiana residents with chronic disease and helps them remain in their own homes as long14 as possible.15 WHEREAS, the study group requests the opportunity to delegate to the public and16 private partnership the work of prioritizing, championing, and stimulating implementation17 of the key recommendations and strategies; and 18 WHEREAS, the study group is confident that the establishment of a public and19 private partnership would focus attention, commitment, and action on improvements in20 health outcomes; and 21 WHEREAS, the study group is requesting authorization to meet quarterly to monitor22 the progress of such improvements. 23 THEREFORE, BE IT RESOLVED that the House of Representatives of the24 Legislature of Louisiana does hereby extend the term of the Study Group on Chronic Care25 Management in the Community and continues the group's responsibilities concerning the26 following:27 (1)Identifying problems related to effective care coordination.28 (2)Assessing current care delivery and support services in Louisiana.29 (3)Examining promising national programmatic models for coordination of care.30 HLS 12RS-1016 ORIGINAL HR NO. 140 Page 3 of 4 (4)Making recommendations for improvements that keep persons with chronic1 diseases in their own homes and communities. 2 BE IT FURTHER RESOLVED that the composition of the study group shall remain3 unchanged.4 BE IT FURTHER RESOLVED that the members of this study group shall continue5 to serve without compensation, except per diem or expense reimbursement to which they6 may be individually entitled as members of their constituent organizations. 7 BE IT FURTHER RESOLVED that the study group may continue under its prior8 organization, adopted rules of procedure, work schedule, and elected chairman and vice9 chairman, and is authorized to adopt any additional measure that it deems necessary for the10 timely performance of its duties.11 BE IT FURTHER RESOLVED that all departments, boards, agencies, officers, and12 institutions of the state and all subdivisions thereof shall continue to cooperate with the study13 group in providing information to assist the study group in carrying out its mission. 14 BE IT FURTHER RESOLVED that the authority of the study group shall expire on15 June 30, 2013. 16 BE IT FURTHER RESOLVED that the study group shall issue a finalized,17 comprehensive written report of its findings to the House of Representatives of the18 Legislature of Louisiana on or before June 30, 2013.19 BE IT FURTHER RESOLVED that a copy of this Resolution shall be transmitted20 to the executive director of the Louisiana Public Health Institute; the executive director of21 the Louisiana Health Care Quality Forum; the executive director of the Pennington22 Biomedical Research Center; the chancellor of the LSU Health Sciences Center; the23 secretary of the Department of Health and Hospitals; the assistant secretary of the office of24 aging and adult services of the Department of Health and Hospitals; the assistant secretary25 of the office of public health of the Department of Health and Hospitals; the director of26 AARP Louisiana; the president and chief executive officer of the Louisiana Hospital27 Association; the chief executive officer of the HomeCare Association of Louisiana; the28 director of the Coalition for Choice to Advance Long Term Care Options in Louisiana; the29 executive vice president of the Louisiana State Medical Society; the president of the Public30 HLS 12RS-1016 ORIGINAL HR NO. 140 Page 4 of 4 Affairs Research Council; the chief financial officer of the Rapides Foundation;
