AN ACT1
To amend and reenact R.S. 37:1202(A)(5), 1203(A)(5), 1207(B), 1213, 1214, and 1230(C),2
and to repeal R.S. 37:1215, relative to the Louisiana Board of Pharmacy; to provide3
with respect to certain qualifications to engage in the practice of pharmacy; to4
require submission of an application by any applicant for licensure by examination;5
to provide with respect to submission of an application by any applicant for licensure6
as a pharmacist by reciprocity; to provide for applications for certain renewals; to7
provide for certain notifications of change; to provide for certain permit renewal8
applications; to remove the requirement to display certain licenses, certificates, and9
registrations; and to provide for related matters.10
Be it enacted by the Legislature of Louisiana:11
Section 1. R.S. 37:1202(A)(5), 1203(A)(5), 1207(B), 1213, 1214, and 1230(C) are12
hereby amended and reenacted to read as follows: 13
§1202.  Qualifications for licensure by examination14
A. Qualifications. To obtain a license to engage in the practice of pharmacy,15
an applicant for licensure by examination shall:16

(5) Have submitted to the board a written an application, in the time frame18
and form prescribed by the board by regulation.19

§1203.  Qualifications for reciprocity21
A.  In order for a pharmacist currently licensed in another jurisdiction to22
obtain a license as a pharmacist by reciprocity in this state, an applicant shall:23

board.26

§1207. Renewal of license, registration, and certification; expiration; reinstatement1

B. The board shall mail make available applications for renewal of licenses,3
registrations, and certificates to each licensed, registered, and certified person by a4
date designated by the board.5

§1213.  Notification of change of business place or employment7
A pharmacist, or pharmacy technician, pharmacy intern, or pharmacy8
technician candidate shall notify the board, in writing, of any change of9
employment within a time frame determined by the board by rule.10
§1214.  Notification of change of address11
A pharmacist, pharmacy technician, or pharmacy intern, or pharmacy12
technician candidate shall notify the board, in writing, of a change of address13
within a time frame determined by the board by rule.14

§1230.  Renewal of permits; expiration; reinstatement16

C. Application for renewal of a permit shall be mailed made available on18
or before a date designated by the board. If a permit holder fails to make application19
to the board for renewal of his permit within a period determined by the board, the20
existing permit shall expire and become null and void.21

Section 2.  R.S. 37:1215 is hereby repealed.23
