HOUSE RESOLUTION NO. 45
BY REPRESENTATIVE HOFFMANN
A RESOLUTION
To commend the Community Provider Association for serving citizens with disabilities and
to recognize the week of April 4, 2016, as Community Provider Association week
at the state capitol.
WHEREAS, services to individuals with disabilities are referred to generally as
"long-term supports and services"; and 
WHEREAS, long-term supports and services include assistance with activities of
daily living such as getting dressed, taking medication, preparing meals, and managing
money; and
WHEREAS, long-term supports and services are critical for individuals with
disabilities to reach their highest potential in terms of health, education, employment, and
overall quality of life; and
WHEREAS, community providers are agencies that provide a spectrum of long-term
supports and services to individuals with disabilities; and
WHEREAS, these supports and services include, without limitation, the following:
(1)  Case management and support coordination.
(2)  In-home supports and services.
(3)  Residential and facility-based supports.
(4)  Day habilitation and community access training.
(5)  Extended follow-along and support in the competitive job market; and

WHEREAS, community providers serve individuals of all ages including newborns
receiving early intervention services, school-age children, transition-age youth, working
adults, and those who are aging; and
WHEREAS, the Community Provider Association was created in 2014 through the
merging of three other associations, and is now the largest organization of its kind in
Louisiana; and
WHEREAS, collectively, the members of the Community Provider Association serve
over twelve thousand individuals with disabilities in this state and employ over ten thousand
Louisiana workers; and
WHEREAS, the Community Provider Association promotes optimal funding,
program integrity, and public policy enabling its members to deliver quality services to
people with disabilities in all community-based service models; and
WHEREAS, the Community Provider Association encourages the professional
growth of community providers and promotes the highest standards of quality and ethics;
and
WHEREAS, the Community Provider Association provides current and relevant
information to its member agencies, and also serves as a vehicle and a forum for the unified
expression of opinions and positions by providers upon matters or events of concern to the
provider community overall; and
WHEREAS, with an increase in the number of children being diagnosed with autism
and an increasing aging population, disability services providers will play an increasingly
significant role in improving the health, education, recreation, and employment opportunities
for Louisiana citizens in the future.
THEREFORE, BE IT RESOLVED that the House of Representatives of the
Legislature of Louisiana does hereby commend the Community Provider Association and
its member organizations for serving citizens with disabilities in our state.
BE IT FURTHER RESOLVED that the House of Representatives of the Legislature
of Louisiana does hereby recognize the week of April 4, 2016, as Community Provider
Association week at the state capitol.

BE IT FURTHER RESOLVED that a suitable copy of this Resolution be transmitted
to the executive director of the Community Provider Association.
