(2)Aformeremployeehastherighttorequest,withinoneyearsubsequentto 89thedateofhisorherseparationfromtheemployer,awrittendescriptionofthequotato 90whichtheemployeewassubjectasofthedateofhisorherseparation,acopyofthe 91employee'sownpersonalwork-speeddataforthesixmonthspriortohisorherdateof HB1550 5 46failstopayanassessmentunderthissection,orunderanyruleunderthissection,after 47ithasbecomeafinalandunappealableorder,orafteracourthasenteredfinal 48judgmentinfavoroftheagency,thedirectorortheattorneygeneralmayinitiate 49collectionproceduresasauthorizedbylaw. 50 9.Ifthedepartmentfindsthataquotaviolatessections285.2000to285.2015,the
