On page 1 of the printed bill, delete lines 11 through 20 and insert:
“(a)(A) Is diagnosed with any type of diabetes by the recipient’s primary care physician or an-
other licensed health care provider whose authorized scope of practice includes the diagnosis of
diabetesand:
“(i) Is treated with insulin; or
“(ii) Has a history of severe hypoglycemia; or
“(B) Is diagnosed with gestational diabetes by the recipient’s primary care physician or another
licensed health care provider whose authorized scope of practice includes the diagnosis of
gestational diabetes; and
“(b) Attends a visit with a health care provider to assess the efficacy of using the continuous
glucose monitor to treat the recipient’s diabetes:
“(A) At least once every six months during the first 18 months after receiving a prescription for
the continuous glucose monitor; and
“(B) At least once every 12 months after the initial 18-month period.”.
