Overview 
Prescribes the information that must be included on the Arizona Department of Health Services 
COVID-19 dashboard. 
History 
Coronaviruses are a family of viruses that can cause illnesses such as the common cold, severe 
acute respiratory syndrome (SARS) and Middle East respiratory syndrome (MERS). A novel 
coronavirus is a new coronavirus that has not been previously identified. The novel coronavirus 
(SARS-CoV-2), has caused a worldwide pandemic of respiratory illness called COVID-19.   
Provisions 
1. Directs the Arizona Department of Health Services to post on their public website's COVID-
19 dashboard a syringe icon with a link entitled Vaccine Injury Data Report. (Sec. 1) 
2. Specifies that the link must contain publicly available national data from the vaccine adverse 
event reporting system regarding injuries reported postvaccination for the COVID-19 
vaccines. (Sec. 1) 
3. Requires the reported data to include the number of reports of all the following: 
a) Permanent disabilities; 
b) Blood clots; 
c) Allergic reactions, including anaphylaxis; 
d) Hospitalizations; 
e) Heart-related illnesses, including heart attacks, myocarditis and pericarditis; and 
f) Death. (Sec. 1) 
4. States that the website must include language that states that the reports are not proof of 
causality. (Sec. 1)
