7878 Be it enacted by the Legislature of the State of Arizona: Section 1. Title 15, chapter 19, article 1, Arizona Revised Statutes, is amended by adding section 15-2406, to read: START_STATUTE15-2406. Arizona empowerment scholarship accounts; quarterly report; definition A. The department of education shall submit a quarterly report, consistent with federal law, to the state board of education, the governor, the president of the senate and the speaker of the house of representatives, shall post a copy on the department's website and shall provide a copy to the secretary of state. The report must include the following: 1. The number of applications received pursuant to section 15-2403, subsection H, whether each application was approved or denied and the justification for each denied application. 2. The number of applications processed within thirty days after receipt and the number of administratively incomplete applications. 3. The number of qualified students disaggregated by: (a) Eligibility category. (b) For qualified students who are eligible pursuant to section 15-2401.01, the number of qualified students who attended a public school in this state in the school year immediately preceding the first year of each qualified student's enrollment in the Arizona empowerment scholarship account program. (c) Grade level. (d) The school district or charter school that each qualified student attended in the school year immediately preceding the first year of each qualified student's enrollment in the Arizona empowerment scholarship account program, if applicable. (e) The types of educational services used, including the amount of expenses approved for each type of educational service. (f) English language learners. (g) Qualified students who are subject to an individualized education program or who have a section 504 accommodation plan. (h) Self-reported race and ethnicity. (i) Household income. 4. The annual award amount associated with each Arizona empowerment scholarship account. 5. The number of qualified students who withdrew from the Arizona empowerment scholarship account program, who failed to renew the Arizona empowerment scholarship account as required by section 15-2402, subsection F or who were removed from the Arizona empowerment scholarship account program pursuant to section 15-2403, and the amount of monies that must be returned to the state by the qualified students described in this paragraph. 6. The amount of approved expenses disaggregated by type of expense as described in section 15-2402, subsection B, paragraph 4. 7. The percentage of qualified schools that are accredited. B. For the purposes of this section, "type of educational services" means either: 1. A qualified school. 2. instruction provided by an approved vendor or provider outside of a qualified school.
