AN ACT

 

amending title 16, chapter 1, article 4, Arizona Revised Statutes, by adding section 16-152.01; relating to qualification and registration of electors.

 Be it enacted by the Legislature of the State of Arizona: Section 1. Title 16, chapter 1, article 4, Arizona Revised Statutes, is amended by adding section 16-152.01, to read: START_STATUTE16-152.01. Standardized voter registration forms; addresses; audits; training; violation  A. notwithstanding any other law, The secretary of state shall develop and maintain a standardized voter registration form to be used by all counties and election officials statewide. The form shall include at least the following: 1. The applicant's full legal name. 2. The applicant's date of birth. 3. The applicant's residential physical address and, if applicable, mailing address, which may be a post office box. 4. The applicant's contact information, including telephone number and optional email address. 5. The applicant's citizenship affirmation. 6. The applicant's declaration of eligibility and signature. B. The form shall also contain clear voter registration deadlines and instructions. The form's format, font and layout shall be consistent across this state, as prescribed by the secretary of state. C. A post office box address is allowed as a mailing address but may not replace the required residential address. The secretary of state shall establish specific formatting guidelines to ensure uniformity in how post office box addresses are submitted and filed in voter registration records. The secretary of state and county recorders shall enforce a standardized format for entering post office box mailing addresses to prevent inconsistencies and clerical errors. D. For persons who do not have a traditional residential address, an alternative descriptive address must be provided in addition to the standardized post office box information. E. The secretary of state and county recorder shall verify compliance with the address requirements prescribed by this section and shall ensure that verification of residency is maintained. F. All counties shall adopt the standardized voter registration form, including the post office box formatting requirements, not later than one year after the effective date of this section.  G. The secretary of state shall conduct regular audits of county voter registration compliance and may take corrective actions as determined by the secretary of state. H. The secretary of state shall provide guidance and training to election officials regarding the proper use and distribution of standardized voter registration forms and post office box mailing address formatting and shall provide voter education materials that include clear instructions on completing the form accurately. I. The secretary of state may include rules and procedures to implement and enforce this section in the instructions and procedures manual issued pursuant to section 16-452. J. a Person who knowingly provides false information, including misrepresentation of a physical address or post office box mailing address address, commits false registration as prescribed by section 16-182.END_STATUTE
