24	24	AN ACT CONCERNING RECOMMENDATIONS BY THE LONG-TERM CARE ADVISORY COUNCIL TO ESTABLISH A SINGLE POINT OF ENTRY FOR LONG-TERM CARE.

26	26	Be it enacted by the Senate and House of Representatives in General Assembly convened:

28	28	Section 1. (NEW) (Effective July 1, 2011) (a) The Bureau of Rehabilitation Services and the Aging Services Division within the Department of Social Services shall establish a state-wide single point of entry system for individuals seeking long-term care. The goal of the single point of entry system shall be to (1) permit any individual seeking long-term care services in the state to obtain information on all long-term care services from single point of entry entities in the individual's geographic service area designated pursuant to subsection (b) of this section, and (2) promote consumer choice of long-term care options.

30	30	(b) The Bureau of Rehabilitation Services and the Aging Services Division, in consultation with the Long-Term Care Advisory Council and members of the public, shall designate geographical service areas for the establishment of single point of entry entities.

32	32	(c) Single point of entry entities designated pursuant to subsection (b) of this section shall:

34	34	(1) Work collaboratively with other designated single point of entry entities in the state to operate the single point of entry system;

36	36	(2) Provide to all individuals seeking information on long-term care, including individuals who pay privately for such care, with information on all long-term care options and services available in the state, including community and home-based care and nursing home care;

38	38	(3) Assess an individual's eligibility for long-term care services and programs in the state, including Medicaid and other public programs and services offered by private and nonprofit organizations, through a comprehensive, uniform screening process;

40	40	(4) Assist individuals in obtaining a timely determination of eligibility from the Department of Social Services for publicly funded long-term care services and programs;

42	42	(5) Assist individuals in developing a long-term care support plan that is person-centered throughout the planning process; and

44	44	(6) Implement quality assurance standards and procedures.

46	46	(d) In addition to the activities required in subsection (c) of this section, the directors of the Bureau of Rehabilitation Services and the Aging Services Division may establish additional requirements, criteria and standards for the operation of single point of entry entities. The directors shall implement a quality assurance program to measure the performance of such designated entities. Any designated entity that fails to comply with the provisions of subsection (c) of this section or any additional requirements, criteria or quality assurance standards established by the directors pursuant to this subsection may be subject to termination as a single point of entry entity.

48	48	Sec. 2. Section 17b-367 of the general statutes is repealed and the following is substituted in lieu thereof (Effective July 1, 2011):

50	50	The Office of Policy and Management, [within existing budgetary resources and] in consultation with the [Select Committee on Aging] joint standing committee of the General Assembly having cognizance of matters relating to aging, the Commission on Aging, personnel designated by the Commissioner of Social Services who administer the CHOICES health insurance assistance program and who administer the single point of entry system established in section 1 of this act and the Long-Term Care Advisory Council, shall develop and maintain a single consumer-oriented Internet web site that provides comprehensive information on long-term care options that are available in Connecticut and information on the single point of entry system established pursuant to section 1 of this act. The web site shall also include direct links and referral information regarding long-term care resources, including private and nonprofit organizations offering advice, counseling and legal services.
