2626 Be it enacted by the Senate and House of Representatives in General Assembly convened:

2828 That section 17a-211 of the general statutes be amended to require the Department of Developmental Services to provide forms that may be filled out electronically for the redetermination of eligibility for services and that the department establish a process and related form for the permanent determination of intellectual disability.
