Explanation 
The bill requires the Department of Public Health (DPH) to 
biennially administer the Connecticut School Health Survey to 
students in grades 9 – 12 if the agency receives funding from the 
federal Center for Disease Control and Prevention for this purpose. 
This provision has no fiscal impact to the state or municipalities.  
In addition, the State Department of Education (SDE), in 
consultation with DPH, must develop for use by a local or regional 
board of education 1) a uniform policy on timely notification to 
parents or guardians about the Connecticut School Health Survey, and 
2) a form to be distributed to parents with specific information about 
the health survey. 
House "A" makes several minor and technical changes that have no 
fiscal impact. 
House "C" adds language requiring SDE to develop a uniform 
policy on notifying parents or guardians about the health survey and
