AN ACT REQUIRING THE INCLUSION AND CONFIRMATION OF
CERTAIN INFORMATION IN MEDICAL RECORDS.
Be it enacted by the Senate and House of Representatives in General
Assembly convened:
That title 19a of the general statutes be amended to require (1) each 1
health care patient chart to include the patient's (A) photograph, (B) date 2
of birth, (C) Social Security number, (D) address, (E) health insurer, and 3
(F) contact information; and (2) each health care provider to ensure that 4
the photograph and other information included in each patient's chart 5
matches the patient presenting to the provider for treatment before 6
billing such patient’s health insurer. 7
Statement of Purpose:
To prevent identity theft in health care settings.
