1019 AN ACT CONCERNING THE PATIENT -CENTERED MEDICAL HOME -
1120 PLUS PROGRAM.
1221 Be it enacted by the Senate and House of Representatives in General

1524 Section 1. (NEW) (Effective from passage) (a) As used in this section, (1) 1
1625 "federally qualified health center" has the same meaning as provided in 2
1726 42 USC 1395x(aa)(4), as amended from time to time, and (2) "Patient-3
1827 Centered Medical Home-Plus program" means a person-centered 4
19-medical program, administered by the Department of Social Services, 5
20-that provides comprehensive and coordinated care to HUSKY Health 6
21-program members under a shared savings model that provides half of 7
22-savings achieved to health care providers who provide quality care at 8
23-reduced costs. 9
24-(b) The Commissioner of Social Services shall pay to a federally 10
25-qualified health center participating in the Patient-Centered Medical 11
26-Home-Plus program an amount equal to the amount such center was 12
27-due under the program in November 2024 and December 2024. The 13
28-commissioner shall not make shared savings payments under the 14
29-Patient-Centered Medical Home-Plus program until such time as the 15
30-arrearage of such payments to a federally qualified health center is 16
