2424 Be It Enacted by the Legislature of the State of Florida: 11

2626 Section 1. Subsection (8) of section 322.08, Florida 13
2727 Statutes, is amended to read: 14
2828 322.08 Application for license; requirements for license 15

3030 (8) The application form for an original, renewal, or 17
3131 replacement driver license or identification card must include 18
3232 language permitting the following: 19
3333 (a) A voluntary contribution of $1 per applicant, which 20
3434 contribution shall be deposited into the Health Care Tru st Fund 21
3535 for organ and tissue donor education and for maintaining the 22
3636 organ and tissue donor registry. 23
3737 (b) A voluntary contribution of $1 per applicant, which 24
3838 shall be distributed to the Florida Council of the Blind. 25

126126 of $1 per applicant to aid the homeless. Contributions made 77
127127 pursuant to this paragraph shall be deposited into the Grants 78
128128 and Donations Trust Fund of the Department of Children and 79
129129 Families and used by the Sta te Office on Homelessness to 80
130130 supplement grants made under s. 420.622(4) and (5), provide 81
131131 information to the public about homelessness in the state, and 82
132132 provide literature for homeless persons seeking assistance. 83

134134 to End Breast Cancer, which shall be distributed to the Florida 85
