21	21	such contributions from the General Revenue Fund 8

24	24	Be It Enacted by the Legislature of the State of Florida: 11

26	26	Section 1. Subsection (8) of section 322.08, Florida 13
27	27	Statutes, is amended to read: 14
28	28	322.08 Application for license; requirements for license 15
29	29	and identification card forms. — 16
30	30	(8) The application form for an original, renewal, or 17
31	31	replacement driver license or identification card must include 18
32	32	language permitting the following: 19
33	33	(a) A voluntary contribution of $1 per applicant, which 20
34	34	contribution shall be deposited into the Health Care Tru st Fund 21
35	35	for organ and tissue donor education and for maintaining the 22
36	36	organ and tissue donor registry. 23
37	37	(b) A voluntary contribution of $1 per applicant, which 24
38	38	shall be distributed to the Florida Council of the Blind. 25

62	62	Heart Disease, which shall be distributed to the Florida Heart 37

65	65	Vision Services, which shall be distributed to the Florida 40
66	66	Association of Agencie s Serving the Blind, Inc., a not -for-41

69	69	services for persons with developmental disabilities, which 44
70	70	shall be distributed to The Arc of Florida. 45
71	71	(j) A voluntary contribution of $1 to the Ronald McDonald 46
72	72	House, which shall be distributed each month to Ronald McDonald 47
73	73	House Charities of Tampa Bay, Inc. 48
74	74	(k) Notwithstanding s. 322.081, a voluntary contribution 49
75	75	of $1 per applicant, which shall be distributed to the League 50

91	91	Prevent Child Sexual Abuse, which shall be distributed to 54
92	92	Lauren's Kids, Inc., a nonprofit organization. 55

94	94	shall be distributed to Prevent Blindness Florida, a not -for-57
95	95	profit organization, to prevent blindness and preserve the sight 58
96	96	of the residents of this state. 59
97	97	(n) Notwithstanding s. 322.081, a voluntary contribution 60
98	98	of $1 per applicant t o the state homes for veterans, to be 61
99	99	distributed on a quarterly basis by the department to the 62
100	100	Operations and Maintenance Trust Fund within the Department of 63
101	101	Veterans' Affairs. 64
102	102	(o) A voluntary contribution of $1 per applicant to the 65
103	103	Disabled American Ve terans, Department of Florida, which shall 66
104	104	be distributed quarterly to Disabled American Veterans, 67
105	105	Department of Florida, a nonprofit organization. 68
106	106	(p) A voluntary contribution of $1 per applicant for 69
107	107	Autism Services and Supports, which shall be distribu ted to 70
108	108	Achievement and Rehabilitation Centers, Inc., Autism Services 71

110	110	(q) A voluntary contribution of $1 per applicant to 73
111	111	Support Our Troops, which shall be distributed to Support Our 74
112	112	Troops, Inc., a Florida not -for-profit organization. 75

125	125	(r) Notwithstanding s. 322.081, a voluntary contribution 76
126	126	of $1 per applicant to aid the homeless. Contributions made 77
127	127	pursuant to this paragraph shall be deposited into the Grants 78
128	128	and Donations Trust Fund of the Department of Children and 79
129	129	Families and used by the Sta te Office on Homelessness to 80
130	130	supplement grants made under s. 420.622(4) and (5), provide 81
131	131	information to the public about homelessness in the state, and 82
132	132	provide literature for homeless persons seeking assistance. 83
133	133	(s) A voluntary contribution of $1 or more per applicant 84
134	134	to End Breast Cancer, which shall be distributed to the Florida 85
135	135	Breast Cancer Foundation. 86
136	136	(t) Notwithstanding s. 322.081(1), a voluntary 87
137	137	contribution of $1 or more per applicant to Childhood Cancer 88
138	138	Care, which shall be distributed to the L ive Like Bella 89
139	139	Childhood Cancer Foundation. 90
140	140	(u) A voluntary contribution of $1 or more per applicant, 91
141	141	which shall be distributed to the Armory Board created under s. 92
142	142	250.40, to be used to maintain, repair, or renovate armories 93
143	143	within this state used for military purposes. 94

145	145	A statement providing an explanation of the purpose of the trust 96
146	146	funds shall also be included. For the purpose of applying the 97
147	147	service charge provided under s. 215.20, contributions received 98
148	148	under paragraphs (b)-(u) (b)-(t) are not income of a revenue 99
