28	28	Be It Enacted by the Legislature of the State of Florida: 15

30	30	Section 1. Paragraph (h) of subsection (2) of section 17
31	31	627.4302, Florida Statutes, is redesignated as paragraph (i), 18
32	32	and a new paragraph (h) is added to that subsection, to read: 19
33	33	627.4302 Identification cards for processing prescription 20
34	34	drug claims.— 21
35	35	(2) Any health insurer or health maintenance organization 22
36	36	and all state and local government entities entering into an 23
37	37	agreement to provide coverage for prescription drugs on an 24
38	38	outpatient basis shall provide a benefits -identification card 25

53	53	state; an indication that the plan is self -funded; or the name 28

56	56	identification card must include the lett ers "FL" on the back 31
57	57	left-hand corner of the card, under which a quick response (QR) 32
58	58	code must be displayed directing the insured or the subscriber 33
59	59	to the consumer complaint website of the Division of Consumer 34
60	60	Services of the department. 35
61	61	2. If the plan is exclusively federally regulated, the 36
62	62	identification card must include the letters "FED" on the back 37
63	63	left-hand corner of the card, under which a QR code must be 38
64	64	displayed directing the insured or the subscriber to the 39
65	65	consumer complaint website of the Cen ters for Medicare and 40
66	66	Medicaid Services. 41

68	68	issued or reissued on or after January 1, 2024. 43

70	70	changes to the consumer complaint website and h otline of the 45

72	72	subscribers who are at an impasse with their insurers or health 47
