2323 Section 1. Paragraphs (b) and (d) of subsection (18) and 10
2424 subsection (19) of section 395.1055, Florida Statutes, are 11
2525 amended to read: 12
2626 395.1055 Rules and enforcement. — 13
2727 (18) In establishing rules for adult cardiovascular 14
2828 services, the agency shall include provisions that allow for: 15
2929 (b)1. For a hospital seeking a Level I program, the 16
3030 hospital must have demonstration that, for the most recent 12-17
3131 month period as reported to the agency, the hospital has 18
3232 provided a minimum of 300 adult inpatient and outpatient 19
3333 diagnostic cardiac catheterizations or, for the most recent 12 -20
3434 month period, has discharged or transferred at least 300 21
3535 patients with the principal diagnosis of ischemic heart disease 22
3636 and that it has a formalized, written transfer agreement with a 23
3737 hospital that has a Level II program, including written 24
3838 transport protocols to ensure safe and efficient transfer of a 25

5252 2.a. A hospital located more than 100 road miles from the 27
5353 closest Level II adult cardiovascular services program is not 28
5454 required to meet the diagnostic cardiac catheterization volume 29
5555 and ischemic heart disease diagnosis volume requirements in 30
5656 subparagraph 1. if the hospital demonstrates that it has, for 31
5757 the most recent 12-month period as reported to the agency, 32
5858 provided a minimum of 100 adult inpatient and outpatient 33
5959 diagnostic cardiac catheterizations or that, for the most recent 34
6060 12-month period, it has discharged or transferred at least 300 35
6161 patients with the principal diagnosis of ischemic heart disease. 36
6262 2.b. A hospital located more than 100 road miles from the 37
6363 closest Level II adult cardiovascular services program does not 38
6464 need to meet the 60-minute transfer time protocol requirement in 39
6565 subparagraph 1. if the hospital demonstrates that it has a 40
6666 formalized, written transfer agreement with a hospital that has 41
6767 a Level II program. The agreement must include written transport 42
6868 protocols to ensure the s afe and efficient transfer of a 43
6969 patient, taking into consideration the patient's clinical and 44
7070 physical characteristics, road and weather conditions, and 45
7171 viability of ground and air ambulance service to transfer the 46
7272 patient. 47
7373 3. At a minimum, the rules for adult cardiovascular 48
7474 services must require nursing and technical staff to have 49
7575 demonstrated experience in handling acutely ill patients 50

8989 experience in dedicated cardiac interventional laboratorie s or 52
9090 surgical centers. If a staff member's previous experience is in 53
9191 a dedicated cardiac interventional laboratory at a hospital that 54
9292 does not have an approved adult open heart surgery program, the 55
9393 staff member's previous experience qualifies only if, at t he 56
9494 time the staff member acquired his or her experience, the 57
9595 dedicated cardiac interventional laboratory: 58
9696 a. Had an annual volume of 500 or more percutaneous 59
9797 cardiac intervention procedures. 60
9898 b. Achieved a demonstrated success rate of 95 percent or 61
9999 greater for percutaneous cardiac intervention procedures. 62
100100 c. Experienced a complication rate of less than 5 percent 63
101101 for percutaneous cardiac intervention procedures. 64
102102 d. Performed diverse cardiac procedures, including, but 65
103103 not limited to, balloon angioplast y and stenting, rotational 66
104104 atherectomy, cutting balloon atheroma remodeling, and procedures 67
105105 relating to left ventricular support capability. 68
106106 (d) Compliance with the most recent guidelines of the 69
107107 American College of Cardiology , and the American Heart 70
108108 Association, the Heart Rhythm Society for hospitals providing 71
109109 electrophysiology services, and the Society for Cardiovascular 72
110110 Angiography and Interventions guidelines for staffing, physician 73
111111 training and experience, operating procedures, equipment, 74
112112 physical plant, and patient selection criteria , to ensure 75
