28 | 28 | | services, the agency shall include provisions that allow for: 15 |

29 | 29 | | (b)1. For a hospital seeking a Level I program, the 16 |
---|
30 | 30 | | hospital must have demonstration that, for the most recent 12-17 |
---|
31 | 31 | | month period as reported to the agency, the hospital has 18 |
---|
32 | 32 | | provided a minimum of 300 adult inpatient and outpatient 19 |
---|
33 | 33 | | diagnostic cardiac catheterizations or, for the most recent 12 -20 |
---|
34 | 34 | | month period, has discharged or transferred at least 300 21 |
---|
35 | 35 | | patients with the principal diagnosis of ischemic heart disease 22 |

36 | 36 | | and that it has a formalized, written transfer agreement with a 23 |
