165165 (1) It is the intent of the Legisla ture that all forms 104

247247 completed to confirm that none of the supplied credentials 162

249249 2. If the voter registration applicant applies using the 164
250250 last four digits of his or her social security number, the 165
251251 department must use all possible means to verify citizenship 166
252252 status and the applicant is required to present his or her 167
253253 social security card before voting for the first time in this 168
254254 state. A voter who presents at the polling place a social 169
255255 security card that contains a work restriction must vote on a 170

281281 sources, including the department's records, to determine if his 184
282282 or her driver license is currently classified by the department 185
283283 as a non-United States citize n license, a surrendered license of 186
284284 a person who moved out of state, or a license of a person who is 187

286286 5. If a supervisor of elections determines that false 189
287287 affirmations have been knowingly made on a voter registration 190
288288 application, the matter mus t be referred to the Office of 191

290290 (b) If a completed voter registration application has been 193
291291 received by the book -closing deadline but the driver license 194
292292 number, the Florida identification card number, or the last four 195
293293 digits of the social security number provided by the applicant 196
294294 cannot be verified, the applicant shall be notified that the 197
295295 number cannot be verified and that the applicant must provide 198

311311 If the applicant provides the necessary evidence, the supervisor 202
312312 shall place the applicant's name on the registration rolls as an 203
313313 active voter. If the applicant has not provided the necessary 204
314314 evidence or the number has not otherwise been verified prior to 205
315315 the applicant presenting himself or herself to vote, the 206
316316 applicant shall be provided a provisional ballot. The 207
317317 provisional ballot shall be c ounted only if the number is 208
318318 verified by the end of the canvassing period or if the applicant 209
319319 presents evidence to the supervisor of elections sufficient to 210
320320 verify the authenticity of the applicant's driver license 211
321321 number, Florida identification card numbe r, or last four digits 212
322322 of the social security number no later than 5 p.m. of the second 213

351351 provides information from which a voter registration offici al 230

353353 exemptions in subsection (4), the voter registration official 232
354354 shall make the notation on the registration records of the 233
355355 statewide voter registration system and the applicant shall not 234
356356 be required to provide the identification required by this 235

358358 (2) The voter registration official shall, upon accepting 237
359359 the voter registration application submitted pursuant to 238
360360 subsection (1), determine if the applicant provided the required 239
361361 identification at the time of registering. If the required 240
362362 identification was not provided, the supervisor shall notify the 241
363363 applicant that he or she must provide one of the following forms 242
364364 of identification before prior to voting the first time in the 243

366366 (3)(a) The following forms of identification shall be 245
