165	165	(1) It is the intent of the Legisla ture that all forms 104
166	166	required to be used in chapters 97 -106 shall be made available 105
167	167	upon request, in alternative formats. Such forms shall include 106
168	168	vote-by-mail ballots as alternative formats for such ballots 107
169	169	become available and the Division of Elections is able to 108
170	170	certify systems that provide them. Whenever possible, such 109
171	171	forms, with the exception of vote -by-mail ballots, shall be made 110

241	241	driver license was obtained using non -United States citizen 156
242	242	documentation, every possible means must be used to determine if 157
243	243	the applicant became a naturalized citizen after obtaining his 158
244	244	or her driver license. The department m ay not accept as valid a 159
245	245	voter registration application if non -United States citizen 160
246	246	status is indicated and shall document the result of checks 161
247	247	completed to confirm that none of the supplied credentials 162
248	248	indicate non-United States citizen status. 163
249	249	2. If the voter registration applicant applies using the 164
250	250	last four digits of his or her social security number, the 165
251	251	department must use all possible means to verify citizenship 166
252	252	status and the applicant is required to present his or her 167
253	253	social security card before voting for the first time in this 168
254	254	state. A voter who presents at the polling place a social 169
255	255	security card that contains a work restriction must vote on a 170
256	256	provisional ballot and provide proof of citizenship before his 171
257	257	or her vote may be counted. 172

276	276	If there are no findings that indicate that the applicant is not 179
277	277	a United States citizen, the application may be accepted as 180

279	279	4. The voter's eligi bility to vote pursuant to s. 97.041 182
280	280	must be verified by comparing information from all available 183
281	281	sources, including the department's records, to determine if his 184
282	282	or her driver license is currently classified by the department 185
283	283	as a non-United States citize n license, a surrendered license of 186
284	284	a person who moved out of state, or a license of a person who is 187
285	285	deceased. 188
286	286	5. If a supervisor of elections determines that false 189
287	287	affirmations have been knowingly made on a voter registration 190
288	288	application, the matter mus t be referred to the Office of 191

290	290	(b) If a completed voter registration application has been 193
291	291	received by the book -closing deadline but the driver license 194
292	292	number, the Florida identification card number, or the last four 195
293	293	digits of the social security number provided by the applicant 196
294	294	cannot be verified, the applicant shall be notified that the 197
295	295	number cannot be verified and that the applicant must provide 198
296	296	evidence to the supervisor sufficient to verify the authenticity 199

311	311	If the applicant provides the necessary evidence, the supervisor 202
312	312	shall place the applicant's name on the registration rolls as an 203
313	313	active voter. If the applicant has not provided the necessary 204
314	314	evidence or the number has not otherwise been verified prior to 205
315	315	the applicant presenting himself or herself to vote, the 206
316	316	applicant shall be provided a provisional ballot. The 207
317	317	provisional ballot shall be c ounted only if the number is 208
318	318	verified by the end of the canvassing period or if the applicant 209
319	319	presents evidence to the supervisor of elections sufficient to 210
320	320	verify the authenticity of the applicant's driver license 211
321	321	number, Florida identification card numbe r, or last four digits 212
322	322	of the social security number no later than 5 p.m. of the second 213

324	324	Section 4. Section 97.0535, Florida Statutes, is amended 215

350	350	time in the state. If the voter registration application clearly 229
351	351	provides information from which a voter registration offici al 230
352	352	can determine that the applicant meets at least one of the 231
353	353	exemptions in subsection (4), the voter registration official 232
354	354	shall make the notation on the registration records of the 233
355	355	statewide voter registration system and the applicant shall not 234
356	356	be required to provide the identification required by this 235

358	358	(2) The voter registration official shall, upon accepting 237
359	359	the voter registration application submitted pursuant to 238
360	360	subsection (1), determine if the applicant provided the required 239
361	361	identification at the time of registering. If the required 240
362	362	identification was not provided, the supervisor shall notify the 241
363	363	applicant that he or she must provide one of the following forms 242
364	364	of identification before prior to voting the first time in the 243
