| 21 | 21 | | WHEREAS, FCPC provides primary care, oral health, mental health, substance abuse5 |

| 22 | 22 | | counseling, specialty care, pharmacy assistance, enabling services, education, and6 |

| 23 | 23 | | comprehensive outreach through service delivery locations in Norcross, Lawrenceville, and7 |
