18 | 18 | | WHEREAS, Project Elevation is a faith-based community-driven health promotion and2 |

19 | 19 | | prevention program that serves the health care needs of rural communities in Southwest3 |

20 | 20 | | Georgia; and4 |

21 | 21 | | WHEREAS, a community health worker is a community-based front line public health5 |
---|
22 | 22 | | worker who is a trusted member, or has an unusually close understanding, of the community6 |
---|
23 | 23 | | served who builds relationships and capacity for health and well-being in underserved areas;7 |
---|

---|
25 | 25 | | WHEREAS, this trusting relationship enables the worker to serve as a liaison, link, or9 |
---|
26 | 26 | | intermediary between health and social services and the community, facilitate access to10 |
---|
27 | 27 | | services, improve the quality and cultural competence of service delivery, build individual11 |
---|
28 | 28 | | and community capacity, and increase health knowledge and self-sufficiency through a range12 |

29 | 29 | | of activities such as outreach, community education, informal counseling, social support, and13 |

37 | 37 | | WHEREAS, Project Elevation has met the goals of improving health literacy, giving18 |

38 | 38 | | participants the power to take their health care decisions into their own hands and make19 |
---|
39 | 39 | | informed decisions, assisting in navigating health care, and working to provide meaningful20 |
---|
40 | 40 | | community-based participatory research; and21 |

41 | 41 | | WHEREAS, Project Elevation trained and enlisted 100 community health workers (identified22 |
---|
42 | 42 | | as Health Champions), using 25 local churches from seven counties throughout Southwest23 |
---|
43 | 43 | | Georgia; and24 |

44 | 44 | | WHEREAS, the program conducts monthly sessions held at each of the 25 churches; and25 |

45 | 45 | | WHEREAS, Project Elevation has screened 945 men for prostate cancer since 2022; and 26 |

46 | 46 | | WHEREAS, these screenings have resulted in identifying 7 percent of the respondents with27 |
---|
47 | 47 | | an elevated PSA of 4.0 or above as well as several individuals who were diagnosed with28 |
---|
48 | 48 | | prostate cancer; and29 |

49 | 49 | | WHEREAS, Project Elevation also served as a conduit to connect 32 percent of the30 |

50 | 50 | | participants with an elevated PSA to a primary care physician who did not have a provider31 |
