| 11 | 11 | | WHEREAS, Georgia Trend magazine evaluates Georgia hospitals that participate in the6 |

| 12 | 12 | | Centers for Medicare and Medicaid Services Hospital Value-Based Purchasing program,7 |

| 13 | 13 | | ranking them by a total performance score derived from clinical outcomes, person and8 |

| 15 | 15 | | WHEREAS, a pillar of healthcare for families in Northeast Georgia for more than 88 years,10 |

| 18 | 18 | | WHEREAS, the hospital's exceptional performance includes retaining 97 percent of13 |

| 19 | 19 | | emergency room patients for the entirety of their care without transfer, achieving a 30014 |

| 32 | 32 | | WHEREAS, it is abundantly fitting and proper that this exemplary healthcare institution be24 |

| 36 | 36 | | Georgia by Georgia Trend magazine, commend its continued dedication to providing28 |
