1313 1 AN ACT concerning revenue.
1414 2 Be it enacted by the People of the State of Illinois,
1515 3 represented in the General Assembly:
1616 4 Section 5. The State Finance Act is amended by adding
1717 5 Section 5.990 and 6z-116 as follows:

1919 7 Sec. 5.990. The Mental Health Income Tax Checkoff Fund.

2121 9 Sec. 6z-116. The Mental Health Income Tax Checkoff Fund;
2222 10 creation. The Mental Health Income Tax Checkoff Fund is
2323 11 created as a special fund in the State treasury. Subject to the
2424 12 appropriation of new moneys made available from the income tax
2525 13 checkoff created under Section 507MMM of the Illinois Income
2626 14 Tax Act, moneys in the Fund shall be used by the Department of
2727 15 Human Services for the purpose of making grants to providers
2828 16 of mental health services in the State.
2929 17 Section 10. The Illinois Income Tax Act is amended by

7171 1 ending on or after December 31, 2023, the Department must
7272 2 print on its standard individual income tax form a provision
7373 3 (i) indicating that if the taxpayer wishes to contribute to
7474 4 the Mental Health Income Tax Checkoff Fund, as authorized by
7575 5 this amendatory Act of the 103rd General Assembly, he or she
7676 6 may do so by stating the amount of the contribution (not less
7777 7 than $1) on the return and (ii) stating that the contribution
7878 8 will reduce the taxpayer's refund or increase the amount of
7979 9 payment to accompany the return. Failure to remit any amount
8080 10 of increased payment shall reduce the contribution
8181 11 accordingly. This Section does not apply to any amended
8282 12 return.
8383 13 Section 99. Effective date. This Act takes effect upon
8484 14 becoming law.
