19 | 19 | | 9 (a) As used in this Section: |

20 | 20 | | 10 "Health care professional" means a physician, registered |

21 | 21 | | 11 professional nurse, or other individual appropriately licensed |
---|
22 | 22 | | 12 or registered to provide health care services. |
---|
23 | 23 | | 13 "Health care provider" means any physician, hospital |
---|
24 | 24 | | 14 facility, facility licensed under the Home Nursing Care Act, |
---|
25 | 25 | | 15 long-term care facility as defined in Section 1-113 of the |
---|
26 | 26 | | 16 Nursing Home Care Act, or other person that is appropriately |
---|
27 | 27 | | 17 licensed or registered to provide health care services. |
---|
28 | 28 | | 18 "Health care provider" does not include independent practice |
---|
29 | 29 | | 19 associations or physician-hospital organizations. |
---|
30 | 30 | | 20 "Patient" means the individual who is receiving health |
---|
31 | 31 | | 21 care services from the health care professional or health care |
---|
32 | 32 | | 22 provider and any individual who is the guarantor of the |
---|
33 | 33 | | 23 payment for such services. |
