11	11	1 AN ACT concerning safety.
12	12	2 Be it enacted by the People of the State of Illinois,
13	13	3 represented in the General Assembly:
14	14	4 Section 5. The Firearm Concealed Carry Act is amended by
15	15	5 changing Section 30 as follows:
16	16	6 (430 ILCS 66/30)
17	17	7 Sec. 30. Contents of license application.
18	18	8 (a) The license application shall be in writing, under
19	19	9 penalty of perjury, on a standard form adopted by the Illinois
20	20	10 State Police and shall be accompanied by the documentation
21	21	11 required in this Section and the applicable fee. Each
22	22	12 application form shall include the following statement printed
23	23	13 in bold type: "Warning: Entering false information on this
24	24	14 form is punishable as perjury under Section 32-2 of the
25	25	15 Criminal Code of 2012."
26	26	16 (b) The application shall contain the following:
27	27	17 (1) the applicant's name, current address, date and
28	28	18 year of birth, place of birth, height, weight, hair color,
29	29	19 eye color, maiden name or any other name the applicant has
30	30	20 used or identified with, and any address where the
31	31	21 applicant resided for more than 30 days within the 10
32	32	22 years preceding the date of the license application;

69	69	2 (3) a waiver of the applicant's privacy and
70	70	3 confidentiality rights and privileges under all federal
71	71	4 and state laws, including those limiting access to
72	72	5 juvenile court, criminal justice, psychological, or
73	73	6 psychiatric records or records relating to any
74	74	7 institutionalization of the applicant, and an affirmative
75	75	8 request that a person having custody of any of these
76	76	9 records provide it or information concerning it to the
77	77	10 Illinois State Police. The waiver only applies to records
78	78	11 sought in connection with determining whether the
79	79	12 applicant qualifies for a license to carry a concealed
80	80	13 firearm under this Act, or whether the applicant remains
81	81	14 in compliance with the Firearm Owners Identification Card
82	82	15 Act;
83	83	16 (4) an affirmation that the applicant possesses a
84	84	17 currently valid Firearm Owner's Identification Card and
85	85	18 card number if possessed or notice the applicant is
86	86	19 applying for a Firearm Owner's Identification Card in
87	87	20 conjunction with the license application;
88	88	21 (5) an affirmation that the applicant has not been
89	89	22 convicted or found guilty of:
90	90	23 (A) a felony;
91	91	24 (B) a misdemeanor involving the use or threat of
92	92	25 physical force or violence to any person within the 5
93	93	26 years preceding the date of the application; or

104	104	1 (C) 2 or more violations related to driving while
105	105	2 under the influence of alcohol, other drug or drugs,
106	106	3 intoxicating compound or compounds, or any combination
107	107	4 thereof, within the 5 years preceding the date of the
108	108	5 license application;
109	109	6 (6) whether the applicant has failed a drug test for a
110	110	7 drug for which the applicant did not have a prescription,
111	111	8 within the previous year, and if so, the provider of the
112	112	9 test, the specific substance involved, and the date of the
113	113	10 test;
114	114	11 (7) written consent for the Illinois State Police to
115	115	12 review and use the applicant's Illinois digital driver's
116	116	13 license or Illinois identification card photograph and
117	117	14 signature;
118	118	15 (8) unless submitted under subsection (a-25) of
119	119	16 Section 4 of the Firearm Owners Identification Card Act, a
120	120	17 full set of fingerprints submitted to the Illinois State
121	121	18 Police in electronic format, provided the Illinois State
122	122	19 Police may accept an application submitted without a set
123	123	20 of fingerprints, in which case the Illinois State Police
124	124	21 shall be granted 30 days in addition to the 90 days
125	125	22 provided under subsection (e) of Section 10 of this Act to
126	126	23 issue or deny a license;
127	127	24 (9) a head and shoulder color photograph in a size
128	128	25 specified by the Illinois State Police taken within the 30
129	129	26 days preceding the date of the license application; and
