1111 1 AN ACT concerning safety.
1212 2 Be it enacted by the People of the State of Illinois,
1313 3 represented in the General Assembly:
1414 4 Section 5. The Firearm Concealed Carry Act is amended by
1515 5 changing Section 30 as follows:

1717 7 Sec. 30. Contents of license application.
1818 8 (a) The license application shall be in writing, under
1919 9 penalty of perjury, on a standard form adopted by the Illinois
2020 10 State Police and shall be accompanied by the documentation
2121 11 required in this Section and the applicable fee. Each
2222 12 application form shall include the following statement printed
2323 13 in bold type: "Warning: Entering false information on this
2424 14 form is punishable as perjury under Section 32-2 of the
2525 15 Criminal Code of 2012."
2626 16 (b) The application shall contain the following:
2727 17 (1) the applicant's name, current address, date and
2828 18 year of birth, place of birth, height, weight, hair color,
2929 19 eye color, maiden name or any other name the applicant has
3030 20 used or identified with, and any address where the
3131 21 applicant resided for more than 30 days within the 10
3232 22 years preceding the date of the license application;

6868 1 valid state identification card number;
6969 2 (3) a waiver of the applicant's privacy and
7070 3 confidentiality rights and privileges under all federal
7171 4 and state laws, including those limiting access to
7272 5 juvenile court, criminal justice, psychological, or
7373 6 psychiatric records or records relating to any
7474 7 institutionalization of the applicant, and an affirmative
7575 8 request that a person having custody of any of these
7676 9 records provide it or information concerning it to the
7777 10 Illinois State Police. The waiver only applies to records
7878 11 sought in connection with determining whether the
7979 12 applicant qualifies for a license to carry a concealed
8080 13 firearm under this Act, or whether the applicant remains
8181 14 in compliance with the Firearm Owners Identification Card
8282 15 Act;
8383 16 (4) an affirmation that the applicant possesses a
8484 17 currently valid Firearm Owner's Identification Card and
8585 18 card number if possessed or notice the applicant is
8686 19 applying for a Firearm Owner's Identification Card in
8787 20 conjunction with the license application;
8888 21 (5) an affirmation that the applicant has not been
8989 22 convicted or found guilty of:
9090 23 (A) a felony;
9191 24 (B) a misdemeanor involving the use or threat of
9292 25 physical force or violence to any person within the 5
9393 26 years preceding the date of the application; or

104104 1 (C) 2 or more violations related to driving while
105105 2 under the influence of alcohol, other drug or drugs,
106106 3 intoxicating compound or compounds, or any combination
107107 4 thereof, within the 5 years preceding the date of the
108108 5 license application;
109109 6 (6) whether the applicant has failed a drug test for a
110110 7 drug for which the applicant did not have a prescription,
111111 8 within the previous year, and if so, the provider of the
112112 9 test, the specific substance involved, and the date of the
113113 10 test;
114114 11 (7) written consent for the Illinois State Police to
115115 12 review and use the applicant's Illinois digital driver's
116116 13 license or Illinois identification card photograph and
117117 14 signature;
118118 15 (8) unless submitted under subsection (a-25) of
119119 16 Section 4 of the Firearm Owners Identification Card Act, a
120120 17 full set of fingerprints submitted to the Illinois State
121121 18 Police in electronic format, provided the Illinois State
122122 19 Police may accept an application submitted without a set
123123 20 of fingerprints, in which case the Illinois State Police
124124 21 shall be granted 30 days in addition to the 90 days
125125 22 provided under subsection (e) of Section 10 of this Act to
126126 23 issue or deny a license;
127127 24 (9) a head and shoulder color photograph in a size
128128 25 specified by the Illinois State Police taken within the 30
129129 26 days preceding the date of the license application; and

140140 1 (10) a photocopy of any certificates or other evidence
141141 2 of compliance with the training requirements under this
