1  HOUSE RESOLUTION
2  WHEREAS, Benjamin Lucas Pantoja, affectionately known as
3  Benny, was born on June 9, 2022 to loving parents Yolanda
4  Carrillo and Rogelio Pantoja; and
5  WHEREAS, Benjamin Pantoja is lovingly remembered by his
6  siblings, Nicolas and Santiago Pantoja; and
7  WHEREAS, Benjamin Pantoja was born 36 weeks and four days
8  before his scheduled due date via c-section; and
9  WHEREAS, Upon requesting a birth certificate for Benjamin
10  Pantoja, his parents were not provided with an official birth
11  certificate or information on how to obtain one, nor were they
12  offered any resources or assistance, but instead received a
13  partially filled out commemorative certificate from the
14  hospital; and
15  WHEREAS, This lack of support and information underscores
16  a significant gap in the resources provided to families who
17  experience such heartbreaking losses and face medical
18  challenges; and
19  WHEREAS, Yolanda Carrillo, driven by her experience with
20  the birth of her son Benjamin Pantoja, has been proactive in

1  advocating for legislative change during the 103rd General
2  Assembly, introducing House Bill 5215 and currently supporting
3  a bill similar to Senate Bill 3182 to address these very
4  issues; and
5  WHEREAS, The introduction of such bills highlights the
6  ongoing need for legislative measures to ensure that families
7  are given proper information and support during these critical
8  times; therefore, be it
9  RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE ONE
10  HUNDRED THIRD GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that
11  we recognize and honor the memory of Benjamin Lucas Pantoja
12  and acknowledge the significant efforts of his mother, Yolanda
13  Carrillo, in advocating for better support and resources for
14  families dealing with medical issues and losses; and be it
