4-AN ACT concerning State government.
5-Be it enacted by the People of the State of Illinois,
6-represented in the General Assembly:
7-Section 5. The Illinois Identification Card Act is amended
8-by changing Sections 4 and 12 as follows:

11-Sec. 4. Identification card.
12-(a) The Secretary of State shall issue a standard Illinois
13-Identification Card to any natural person who is a resident of
14-the State of Illinois who applies for such card, or renewal
15-thereof. No identification card shall be issued to any person
16-who holds a valid foreign state identification card, license,
17-or permit unless the person first surrenders to the Secretary
18-of State the valid foreign state identification card, license,
19-or permit. The card shall be prepared and supplied by the
20-Secretary of State and shall include a photograph and
21-signature or mark of the applicant. However, the Secretary of
22-State may provide by rule for the issuance of Illinois
23-Identification Cards without photographs if the applicant has
24-a bona fide religious objection to being photographed or to
25-the display of his or her photograph. The Illinois
26-Identification Card may be used for identification purposes in

4+2 Be it enacted by the People of the State of Illinois,

6+4 Section 5. The Illinois Identification Card Act is amended

10+8 Sec. 4. Identification card.
11+9 (a) The Secretary of State shall issue a standard Illinois
12+10 Identification Card to any natural person who is a resident of
13+11 the State of Illinois who applies for such card, or renewal
14+12 thereof. No identification card shall be issued to any person
15+13 who holds a valid foreign state identification card, license,
16+14 or permit unless the person first surrenders to the Secretary

34-As used in this Act, "photograph" means any color photograph
35-or digitally produced and captured image of an applicant for
36-an identification card. As used in this Act, "signature" means
37-the name of a person as written by that person and captured in
38-a manner acceptable to the Secretary of State.
39-(a-5) If an applicant for an identification card has a
40-current driver's license or instruction permit issued by the
41-Secretary of State, the Secretary may require the applicant to
42-utilize the same residence address and name on the
43-identification card, driver's license, and instruction permit
44-records maintained by the Secretary. The Secretary may
45-promulgate rules to implement this provision.
46-(a-10) If the applicant is a judicial officer as defined
47-in Section 1-10 of the Judicial Privacy Act or a peace officer,
48-the applicant may elect to have his or her office or work
49-address listed on the card instead of the applicant's
50-residence or mailing address. The Secretary may promulgate
51-rules to implement this provision. For the purposes of this
52-subsection (a-10), "peace officer" means any person who by
53-virtue of his or her office or public employment is vested by
54-law with a duty to maintain public order or to make arrests for
55-a violation of any penal statute of this State, whether that
56-duty extends to all violations or is limited to specific
57-violations.
58-(a-15) The Secretary of State may provide for an expedited

62-The Secretary shall charge an additional fee for the expedited
63-issuance of an Illinois Identification Card, to be set by
64-rule, not to exceed $75. All fees collected by the Secretary
65-for expedited Illinois Identification Card service shall be
66-deposited into the Secretary of State Special Services Fund.
67-The Secretary may adopt rules regarding the eligibility,
68-process, and fee for an expedited Illinois Identification
69-Card. If the Secretary of State determines that the volume of
70-expedited identification card requests received on a given day
71-exceeds the ability of the Secretary to process those requests
72-in an expedited manner, the Secretary may decline to provide
73-expedited services, and the additional fee for the expedited
74-service shall be refunded to the applicant.
75-(a-20) The Secretary of State shall issue a standard
76-Illinois Identification Card to a person committed to the
77-Department of Corrections, the or Department of Juvenile
78-Justice, a Federal Bureau of Prisons facility located in
79-Illinois, or a county jail or county department of corrections
80-as follows: upon receipt of the person's birth certificate,
81-social security card, photograph, proof of residency upon
82-discharge, and an identification card application transferred
83-via a secure method as agreed upon by the Secretary and the
84-Department of Corrections or Department of Juvenile Justice.
85-Illinois residency shall be established by submission of a
86-Secretary of State prescribed Identification Card verification

89-form completed by the respective Department.
90-(1) A committed person who has previously held an
91-Illinois Identification Card or an Illinois driver's
92-license shall submit an Identification Card verification
93-form to the Secretary of State, including a photograph
94-taken by the correctional facility, proof of residency
95-upon discharge, and a social security number, if the
96-committed person has a social security number. If the
97-committed person does not have a social security number
98-and is eligible for a social security number, the
99-Secretary of State shall not issue a standard Illinois
100-Identification Card until the committed person obtains a
101-social security number. If the committed person's
102-photograph and demographic information matches an existing
103-Illinois Identification Card or Illinois driver's license
104-and the Secretary of State verifies the applicant's social
105-security number with the Social Security Administration,
106-the Secretary of State shall issue the committed person a
107-standard Illinois Identification Card. If the photograph
108-or demographic information matches an existing Illinois
109-Identification Card or Illinois driver's license in
110-another person's name or identity, a standard Illinois
111-Identification Card shall not be issued until the
112-committed person submits a certified birth certificate and
113-social security card to the Secretary of State and the
114-Secretary of State verifies the identity of the committed

117-person. If the Secretary of State cannot find a match to an
118-existing Illinois Identification Card or Illinois driver's
119-license, the committed person may apply for a standard
120-Illinois Identification card as described in paragraph
121-(2).
122-(2) A committed person who has not previously held an
123-Illinois Identification Card or Illinois driver's license
124-or for whom a match cannot be found as described in
125-paragraph (1) shall submit an Identification Card
126-verification form, including a photograph taken by the
127-correctional facility, a certified birth certificate,
128-proof of residency upon discharge, and a social security
129-number, if the committed has a social security number. If
130-the committed person does not have a social security
131-number and is eligible for a social security number, the
132-Secretary of State shall not issue a standard Illinois
133-Identification Card until the committed person obtains a
134-social security number. If the Secretary of State verifies
135-the applicant's social security number with the Social
136-Security Administration, the Secretary of State shall
137-issue the committed person a standard Illinois
138-Identification Card.
139-The Illinois Identification Card verification form
140-described in this subsection shall be prescribed by the
141-Secretary State. The Secretary of State and correctional
142-facilities in this State shall establish a secure method to

146-(a-25) The Secretary of State shall issue a limited-term

149-aftercare release, final discharge, or pardon from the
150-Department of Corrections, the or Department of Juvenile
151-Justice, a Federal Bureau of Prisons facility located in
152-Illinois, or a county jail or county department of
153-corrections, if the released person does not obtain a standard
154-Illinois Identification Card as described in subsection (a-20)
155-prior to release is unable to present a certified copy of his
156-or her birth certificate and social security card or other
157-documents authorized by the Secretary, but does present a
158-Secretary of State prescribed Identification Card verification
159-form completed by the correctional facility Department of
160-Corrections or Department of Juvenile Justice, verifying the

163-Illinois residence address. The verification form must have

167-limited-term Illinois Identification Card, if the released
168-person submits to the Secretary of State a certified copy of
169-his or her birth certificate and his or her social security

173-standard Illinois Identification Card shall be issued. A
174-limited-term Illinois Identification Card may not be renewed.
175-This subsection shall not apply to a released person who
176-was unable to obtain a standard Illinois Identification Card
177-because his or her photograph or demographic information
178-matched an existing Illinois Identification Card or Illinois
179-driver's license in another person's name or identity or to a
180-released person who does not have a social security number and
181-is eligible for a social security number.
182-(a-30) The Secretary of State shall issue a standard
183-Illinois Identification Card to a person upon conditional
184-release or absolute discharge from the custody of the
185-Department of Human Services, if the person presents a
186-certified copy of his or her birth certificate, social
187-security card, or other documents authorized by the Secretary,
188-and a document proving his or her Illinois residence address.
189-The Secretary of State shall issue a standard Illinois
190-Identification Card to a person prior to his or her
191-conditional release or absolute discharge if personnel from
192-the Department of Human Services bring the person to a
193-Secretary of State location with the required documents.
194-Documents proving residence address may include any official
195-document of the Department of Human Services showing the
196-person's address after release and a Secretary of State
197-prescribed verification form, which may be executed by
198-personnel of the Department of Human Services.

201-(a-35) The Secretary of State shall issue a limited-term
202-Illinois Identification Card valid for 90 days to a person
203-upon conditional release or absolute discharge from the

205-unable to present a certified copy of his or her birth

207-authorized by the Secretary, but does present a Secretary of

220-any official document of the Department of Human Services
221-showing the person's address after release and a Secretary of
222-State prescribed verification form, which may be executed by
223-personnel of the Department of Human Services.
224-(b) The Secretary of State shall issue a special Illinois
225-Identification Card, which shall be known as an Illinois
226-Person with a Disability Identification Card, to any natural

242-comprehensible designation of the type and classification of

244-Act. However, the Secretary of State may provide by rule for
245-the issuance of Illinois Person with a Disability
246-Identification Cards without photographs if the applicant has
247-a bona fide religious objection to being photographed or to
248-the display of his or her photograph. If the applicant so
249-requests, the card shall include a description of the
250-applicant's disability and any information about the
251-applicant's disability or medical history which the Secretary
252-determines would be helpful to the applicant in securing
253-emergency medical care. If a mark is used in lieu of a
254-signature, such mark shall be affixed to the card in the

257-presence of two witnesses who attest to the authenticity of
258-the mark. The Illinois Person with a Disability Identification
259-Card may be used for identification purposes in any lawful
260-situation by the person to whom it was issued.
261-The Illinois Person with a Disability Identification Card
262-may be used as adequate documentation of disability in lieu of
263-a physician's determination of disability, a determination of
264-disability from a physician assistant, a determination of
265-disability from an advanced practice registered nurse, or any
266-other documentation of disability whenever any State law
267-requires that a person with a disability provide such
268-documentation of disability, however an Illinois Person with a
269-Disability Identification Card shall not qualify the
270-cardholder to participate in any program or to receive any
271-benefit which is not available to all persons with like
272-disabilities. Notwithstanding any other provisions of law, an
273-Illinois Person with a Disability Identification Card, or
274-evidence that the Secretary of State has issued an Illinois
275-Person with a Disability Identification Card, shall not be
276-used by any person other than the person named on such card to
277-prove that the person named on such card is a person with a
278-disability or for any other purpose unless the card is used for
279-the benefit of the person named on such card, and the person
280-named on such card consents to such use at the time the card is
281-so used.
282-An optometrist's determination of a visual disability

285-under Section 4A of this Act is acceptable as documentation
286-for the purpose of issuing an Illinois Person with a
287-Disability Identification Card.
288-When medical information is contained on an Illinois
289-Person with a Disability Identification Card, the Office of
290-the Secretary of State shall not be liable for any actions
291-taken based upon that medical information.
292-(c) The Secretary of State shall provide that each
293-original or renewal Illinois Identification Card or Illinois
294-Person with a Disability Identification Card issued to a
295-person under the age of 21 shall be of a distinct nature from
296-those Illinois Identification Cards or Illinois Person with a
297-Disability Identification Cards issued to individuals 21 years
298-of age or older. The color designated for Illinois
299-Identification Cards or Illinois Person with a Disability
300-Identification Cards for persons under the age of 21 shall be
301-at the discretion of the Secretary of State.
302-(c-1) Each original or renewal Illinois Identification
303-Card or Illinois Person with a Disability Identification Card
304-issued to a person under the age of 21 shall display the date
305-upon which the person becomes 18 years of age and the date upon
306-which the person becomes 21 years of age.
307-(c-3) The General Assembly recognizes the need to identify
308-military veterans living in this State for the purpose of
309-ensuring that they receive all of the services and benefits to
310-which they are legally entitled, including healthcare,

144+3 license, the committed person may apply for a standard
145+4 Illinois Identification card as described in paragraph
146+5 (2).
147+6 (2) A committed person who has not previously held an

149+8 or for whom a match cannot be found as described in
150+9 paragraph (1) shall submit an Identification Card
151+10 verification form, including a photograph taken by the
152+11 correctional facility, a certified birth certificate,

154+13 number, if the committed has a social security number. If
155+14 the committed person does not have a social security
156+15 number and is eligible for a social security number, the
157+16 Secretary of State shall not issue a standard Illinois
158+17 Identification Card until the committed person obtains a
159+18 social security number. If the Secretary of State verifies
160+19 the applicant's social security number with the Social
161+20 Security Administration, the Secretary of State shall
162+21 issue the committed person a standard Illinois
163+22 Identification Card.
164+23 The Illinois Identification Card verification form
165+24 described in this subsection shall be prescribed by the
166+25 Secretary State. The Secretary of State and correctional
167+26 facilities in this State shall establish a secure method to

313-education assistance, and job placement. To assist the State
314-in identifying these veterans and delivering these vital
315-services and benefits, the Secretary of State is authorized to
316-issue Illinois Identification Cards and Illinois Person with a
317-Disability Identification Cards with the word "veteran"
318-appearing on the face of the cards. This authorization is
319-predicated on the unique status of veterans. The Secretary may
320-not issue any other identification card which identifies an
321-occupation, status, affiliation, hobby, or other unique
322-characteristics of the identification card holder which is
323-unrelated to the purpose of the identification card.
324-(c-5) Beginning on or before July 1, 2015, the Secretary
325-of State shall designate a space on each original or renewal
326-identification card where, at the request of the applicant,
327-the word "veteran" shall be placed. The veteran designation
328-shall be available to a person identified as a veteran under
329-subsection (b) of Section 5 of this Act who was discharged or
330-separated under honorable conditions.
331-(d) The Secretary of State may issue a Senior Citizen
332-discount card, to any natural person who is a resident of the
333-State of Illinois who is 60 years of age or older and who
334-applies for such a card or renewal thereof. The Secretary of
335-State shall charge no fee to issue such card. The card shall be
336-issued in every county and applications shall be made
337-available at, but not limited to, nutrition sites, senior
338-citizen centers and Area Agencies on Aging. The applicant,

341-upon receipt of such card and prior to its use for any purpose,
342-shall have affixed thereon in the space provided therefor his
343-signature or mark.
344-(e) The Secretary of State, in his or her discretion, may
345-designate on each Illinois Identification Card or Illinois
346-Person with a Disability Identification Card a space where the
347-card holder may place a sticker or decal, issued by the
348-Secretary of State, of uniform size as the Secretary may
349-specify, that shall indicate in appropriate language that the
350-card holder has renewed his or her Illinois Identification
351-Card or Illinois Person with a Disability Identification Card.

399-(a-15) The Secretary of State may provide for an expedited
400-process for the issuance of an Illinois Identification Card.
401-The Secretary shall charge an additional fee for the expedited
402-issuance of an Illinois Identification Card, to be set by
403-rule, not to exceed $75. All fees collected by the Secretary
404-for expedited Illinois Identification Card service shall be
405-deposited into the Secretary of State Special Services Fund.
406-The Secretary may adopt rules regarding the eligibility,
407-process, and fee for an expedited Illinois Identification
408-Card. If the Secretary of State determines that the volume of
409-expedited identification card requests received on a given day
410-exceeds the ability of the Secretary to process those requests

414-(a-20) The Secretary of State shall issue a standard

628-An optometrist's determination of a visual disability
629-under Section 4A of this Act is acceptable as documentation
630-for the purpose of issuing an Illinois Person with a
631-Disability Identification Card.
632-When medical information is contained on an Illinois
633-Person with a Disability Identification Card, the Office of
634-the Secretary of State shall not be liable for any actions
635-taken based upon that medical information.
636-(c) The Secretary of State shall provide that each
637-original or renewal Illinois Identification Card or Illinois
638-Person with a Disability Identification Card issued to a
639-person under the age of 21 shall be of a distinct nature from
640-those Illinois Identification Cards or Illinois Person with a
641-Disability Identification Cards issued to individuals 21 years

650-issued to a person under the age of 21 shall display the date
651-upon which the person becomes 18 years of age and the date upon
652-which the person becomes 21 years of age.
653-(c-3) The General Assembly recognizes the need to identify
654-military veterans living in this State for the purpose of
655-ensuring that they receive all of the services and benefits to
656-which they are legally entitled, including healthcare,
657-education assistance, and job placement. To assist the State
658-in identifying these veterans and delivering these vital
659-services and benefits, the Secretary of State is authorized to
660-issue Illinois Identification Cards and Illinois Person with a
661-Disability Identification Cards with the word "veteran"
662-appearing on the face of the cards. This authorization is
663-predicated on the unique status of veterans. The Secretary may
664-not issue any other identification card which identifies an
665-occupation, status, affiliation, hobby, or other unique
666-characteristics of the identification card holder which is
667-unrelated to the purpose of the identification card.
668-(c-5) Beginning on or before July 1, 2015, the Secretary
669-of State shall designate a space on each original or renewal
670-identification card where, at the request of the applicant,
671-the word "veteran" shall be placed. The veteran designation
672-shall be available to a person identified as a veteran under
673-subsection (b) of Section 5 of this Act who was discharged or

677-(d) The Secretary of State may issue a Senior Citizen
678-discount card, to any natural person who is a resident of the
679-State of Illinois who is 60 years of age or older and who
680-applies for such a card or renewal thereof. The Secretary of
681-State shall charge no fee to issue such card. The card shall be
682-issued in every county and applications shall be made
683-available at, but not limited to, nutrition sites, senior
684-citizen centers and Area Agencies on Aging. The applicant,
685-upon receipt of such card and prior to its use for any purpose,
686-shall have affixed thereon in the space provided therefor his
687-signature or mark.
688-(e) The Secretary of State, in his or her discretion, may
689-designate on each Illinois Identification Card or Illinois
690-Person with a Disability Identification Card a space where the
691-card holder may place a sticker or decal, issued by the
692-Secretary of State, of uniform size as the Secretary may
693-specify, that shall indicate in appropriate language that the
694-card holder has renewed his or her Illinois Identification
695-Card or Illinois Person with a Disability Identification Card.

395+2 in identifying these veterans and delivering these vital
396+3 services and benefits, the Secretary of State is authorized to
397+4 issue Illinois Identification Cards and Illinois Person with a
398+5 Disability Identification Cards with the word "veteran"
399+6 appearing on the face of the cards. This authorization is
400+7 predicated on the unique status of veterans. The Secretary may
401+8 not issue any other identification card which identifies an
402+9 occupation, status, affiliation, hobby, or other unique
403+10 characteristics of the identification card holder which is

405+12 (c-5) Beginning on or before July 1, 2015, the Secretary

444+15 (a) The Secretary of State shall issue a standard Illinois
445+16 Identification Card to any natural person who is a resident of

467+3 the display of his or her photograph. The Illinois
468+4 Identification Card may be used for identification purposes in
469+5 any lawful situation only by the person to whom it was issued.
470+6 As used in this Act, "photograph" means any color photograph
471+7 or digitally produced and captured image of an applicant for
472+8 an identification card. As used in this Act, "signature" means
473+9 the name of a person as written by that person and captured in
474+10 a manner acceptable to the Secretary of State.
475+11 (a-5) If an applicant for an identification card has a
476+12 current driver's license or instruction permit issued by the
477+13 Secretary of State, the Secretary may require the applicant to
478+14 utilize the same residence address and name on the
479+15 identification card, driver's license, and instruction permit
480+16 records maintained by the Secretary. The Secretary may
481+17 promulgate rules to implement this provision.
482+18 (a-10) If the applicant is a judicial officer as defined
483+19 in Section 1-10 of the Judicial Privacy Act or a peace officer,
484+20 the applicant may elect to have his or her office or work
485+21 address listed on the card instead of the applicant's
486+22 residence or mailing address. The Secretary may promulgate
487+23 rules to implement this provision. For the purposes of this
488+24 subsection (a-10), "peace officer" means any person who by
489+25 virtue of his or her office or public employment is vested by

504+4 (a-15) The Secretary of State may provide for an expedited
505+5 process for the issuance of an Illinois Identification Card.
506+6 The Secretary shall charge an additional fee for the expedited
507+7 issuance of an Illinois Identification Card, to be set by
508+8 rule, not to exceed $75. All fees collected by the Secretary
509+9 for expedited Illinois Identification Card service shall be
510+10 deposited into the Secretary of State Special Services Fund.
511+11 The Secretary may adopt rules regarding the eligibility,
512+12 process, and fee for an expedited Illinois Identification
513+13 Card. If the Secretary of State determines that the volume of
514+14 expedited identification card requests received on a given day
515+15 exceeds the ability of the Secretary to process those requests
516+16 in an expedited manner, the Secretary may decline to provide
517+17 expedited services, and the additional fee for the expedited
518+18 service shall be refunded to the applicant.
519+19 (a-20) The Secretary of State shall issue a standard
520+20 Illinois Identification Card to a person committed to the
521+21 Department of Corrections, the or Department of Juvenile

726+10 person who is a resident of the State of Illinois, who is a
727+11 person with a disability as defined in Section 4A of this Act,
728+12 who applies for such card, or renewal thereof. No Illinois
729+13 Person with a Disability Identification Card shall be issued
730+14 to any person who holds a valid foreign state identification
731+15 card, license, or permit unless the person first surrenders to
732+16 the Secretary of State the valid foreign state identification
733+17 card, license, or permit. The Secretary of State shall charge
734+18 no fee to issue such card. The card shall be prepared and
735+19 supplied by the Secretary of State, and shall include a
736+20 photograph and signature or mark of the applicant, a

753+1 Identification Cards without photographs if the applicant has
754+2 a bona fide religious objection to being photographed or to
755+3 the display of his or her photograph. If the applicant so
756+4 requests, the card shall include a description of the
757+5 applicant's disability and any information about the
758+6 applicant's disability or medical history which the Secretary
759+7 determines would be helpful to the applicant in securing
760+8 emergency medical care. If a mark is used in lieu of a
761+9 signature, such mark shall be affixed to the card in the
762+10 presence of two witnesses who attest to the authenticity of
763+11 the mark. The Illinois Person with a Disability Identification
764+12 Card may be used for identification purposes in any lawful
765+13 situation by the person to whom it was issued.
766+14 The Illinois Person with a Disability Identification Card
767+15 may be used as adequate documentation of disability in lieu of
768+16 a physician's determination of disability, a determination of
769+17 disability from a physician assistant, a determination of
770+18 disability from an advanced practice registered nurse, or any
771+19 other documentation of disability whenever any State law
772+20 requires that a person with a disability provide such

789+1 evidence that the Secretary of State has issued an Illinois
790+2 Person with a Disability Identification Card, shall not be
791+3 used by any person other than the person named on such card to
792+4 prove that the person named on such card is a person with a
793+5 disability or for any other purpose unless the card is used for
794+6 the benefit of the person named on such card, and the person
795+7 named on such card consents to such use at the time the card is
796+8 so used.
797+9 An optometrist's determination of a visual disability
798+10 under Section 4A of this Act is acceptable as documentation
799+11 for the purpose of issuing an Illinois Person with a
800+12 Disability Identification Card.
801+13 When medical information is contained on an Illinois
802+14 Person with a Disability Identification Card, the Office of
803+15 the Secretary of State shall not be liable for any actions
804+16 taken based upon that medical information.
805+17 (c) The Secretary of State shall provide that each
806+18 original or renewal Illinois Identification Card or Illinois
807+19 Person with a Disability Identification Card issued to a
808+20 person under the age of 21 shall be of a distinct nature from
809+21 those Illinois Identification Cards or Illinois Person with a
810+22 Disability Identification Cards issued to individuals 21 years
811+23 of age or older. The color designated for Illinois
812+24 Identification Cards or Illinois Person with a Disability
813+25 Identification Cards for persons under the age of 21 shall be
814+26 at the discretion of the Secretary of State.

827+3 issued to a person under the age of 21 shall display the date
828+4 upon which the person becomes 18 years of age and the date upon
829+5 which the person becomes 21 years of age.
830+6 (c-3) The General Assembly recognizes the need to identify
831+7 military veterans living in this State for the purpose of
832+8 ensuring that they receive all of the services and benefits to
833+9 which they are legally entitled, including healthcare,
834+10 education assistance, and job placement. To assist the State
835+11 in identifying these veterans and delivering these vital
836+12 services and benefits, the Secretary of State is authorized to
837+13 issue Illinois Identification Cards and Illinois Person with a
838+14 Disability Identification Cards with the word "veteran"
839+15 appearing on the face of the cards. This authorization is
840+16 predicated on the unique status of veterans. The Secretary may
841+17 not issue any other identification card which identifies an
842+18 occupation, status, affiliation, hobby, or other unique
843+19 characteristics of the identification card holder which is
844+20 unrelated to the purpose of the identification card.
845+21 (c-5) Beginning on or before July 1, 2015, the Secretary
846+22 of State shall designate a space on each original or renewal
847+23 identification card where, at the request of the applicant,
848+24 the word "veteran" shall be placed. The veteran designation
849+25 shall be available to a person identified as a veteran under
850+26 subsection (b) of Section 5 of this Act who was discharged or

862+2 (d) The Secretary of State may issue a Senior Citizen
863+3 discount card, to any natural person who is a resident of the
864+4 State of Illinois who is 60 years of age or older and who
865+5 applies for such a card or renewal thereof. The Secretary of
866+6 State shall charge no fee to issue such card. The card shall be
867+7 issued in every county and applications shall be made
868+8 available at, but not limited to, nutrition sites, senior
869+9 citizen centers and Area Agencies on Aging. The applicant,
870+10 upon receipt of such card and prior to its use for any purpose,
871+11 shall have affixed thereon in the space provided therefor his
872+12 signature or mark.
873+13 (e) The Secretary of State, in his or her discretion, may
874+14 designate on each Illinois Identification Card or Illinois

876+16 card holder may place a sticker or decal, issued by the
