22 | 22 | | 10 protected under the federal Protection of Pupil Rights |

23 | 23 | | 11 Amendment of 1978. |

24 | 24 | | 12 (b) If a school district administers a self-report survey |

25 | 25 | | 13 to its students that may reveal personal, protected |

26 | 26 | | 14 information, no student under the age of 18 is required to take |

27 | 27 | | 15 or participate in the self-report survey unless parental or |

28 | 28 | | 16 guardian consent is given in writing. Prior to the self-report |

29 | 29 | | 17 survey being administered, a parent or guardian shall be |
