Requires a school district that administers a self-report survey to its students that may reveal personal, protected information to obtain parental or guardian consent in writing if the student is under the age of 18 before the student may participate in the self-report survey. Requires a school district to afford a parent or guardian the opportunity to review, either electronically or in person, the scope of the questions to be asked. Provides that refusal to grant consent for a student to participate in the self-report survey shall not be a reason for disciplinary action, academic penalty, suspension, or expulsion or any other sanction of the student. 13	13	1 AN ACT concerning education.
14	14	2 Be it enacted by the People of the State of Illinois,
15	15	3 represented in the General Assembly:
16	16	4 Section 5. The School Code is amended by adding Section
17	17	5 10-31 as follows:
18	18	6 (105 ILCS 5/10-31 new)
19	19	7 Sec. 10-31. Self-report surveys.
20	20	8 (a) For purposes of this Section, "personal, protected
21	21	9 information" means any information related to any topic
22	22	10 protected under the federal Protection of Pupil Rights
23	23	11 Amendment of 1978.
24	24	12 (b) If a school district administers a self-report survey
25	25	13 to its students that may reveal personal, protected
26	26	14 information, no student under the age of 18 is required to take
27	27	15 or participate in the self-report survey unless parental or
28	28	16 guardian consent is given in writing. Prior to the self-report
29	29	17 survey being administered, a parent or guardian shall be
30	30	18 afforded the opportunity to review, either electronically or
31	31	19 in person, the scope of the questions to be asked. Refusal to
32	32	20 grant consent for a student to participate in the self-report
33	33	21 survey shall not be a reason for disciplinary action, academic
34	34	22 penalty, suspension, or expulsion or any other sanction of the
35	35	23 student.
