1212 2 Be it enacted by the People of the State of Illinois,
1313 3 represented in the General Assembly:
1414 4 Section 5. The Election Code is amended by changing

1717 7 Sec. 19-3. Application for a vote by mail ballot.
1818 8 (a) The application for a vote by mail ballot for a single
1919 9 election shall be substantially in the following form:
2020 10 APPLICATION FOR VOTE BY MAIL BALLOT
2121 11 To be voted at the .... election in the County of .... and
2222 12 State of Illinois.
2323 13 I state that I am a resident of .... in the municipality of
2424 14 .... in the county of ....; that I have resided at such address
2525 15 for at least 30 days; that I am lawfully entitled to vote at
2626 16 the .... election to be held on ....; and that I wish to vote
2727 17 by mail.
2828 18 I hereby make application for an official ballot or

159159 20 party affiliation for a primary election ballot by a method
160160 21 and deadline published and selected by the election authority.
161161 22 (d) If application is made electronically, the applicant
162162 23 shall mark the box associated with the above described
163163 24 statement included as part of the online application

177177 2 (e) Any person may produce, reproduce, distribute, or
178178 3 return to an election authority an application under this
179179 4 Section. If applications are sent to a post office box
180180 5 controlled by any individual or organization that is not an
181181 6 election authority, those applications shall (i) include a
182182 7 valid and current phone number for the individual or
183183 8 organization controlling the post office box and (ii) be
184184 9 turned over to the appropriate election authority within 7
185185 10 days of receipt or, if received within 2 weeks of the election
186186 11 in which an applicant intends to vote, within 2 days of
187187 12 receipt. Failure to turn over the applications in compliance
188188 13 with this paragraph shall constitute a violation of this Code
189189 14 and shall be punishable as a petty offense with a fine of $100
190190 15 per application. Removing, tampering with, or otherwise
191191 16 knowingly making the postmark on the application unreadable by
192192 17 the election authority shall establish a rebuttable
193193 18 presumption of a violation of this paragraph. Upon receipt,
194194 19 the appropriate election authority shall accept and promptly
195195 20 process any application under this Section submitted in a form
196196 21 substantially similar to that required by this Section,
197197 22 including any substantially similar production or reproduction
198198 23 generated by the applicant.
199199 24 (f) An election authority may combine the applications in
200200 25 subsections (a) and (b) onto one form, but the distinction
