HOUSE RESOLUTION NO. 140
BY REPRESENTATIVE SIMON
A RESOLUTION
To extend the work of the Study Group on Chronic Care Management in the Community,
herein referred to as the "study group", to study and make recommendations for
improving the coordination of care and support services so that persons with chronic
diseases can remain in their homes and communities as long as possible.
WHEREAS, by House Resolution No. 125 of the 2011 Regular Session of the
Legislature, the House of Representatives created the Study Group on Chronic Care
Management in the Community, thereby recognizing the need to identify solutions for
improving the delivery of chronic care in the community; and
WHEREAS, the House of Representatives also recognized the need to improve
Louisiana's chronic care delivery in the areas of housing, transportation, support services,
and technology; and
WHEREAS, the House of Representatives has received an informative preliminary
report from the study group which addressed key recommendations and strategies for
improving the coordination of care and support services for persons with chronic diseases;
and
WHEREAS, the report included eight key recommendations and strategies based on
the need to coordinate care differently when addressing people with chronic diseases, which
include the following:
(1)Ensuring seamless transitions between care settings.
(2)Improving access to coordination of care, including both health- and
community-based services.
(3)Utilizing current and emerging technology to improve care, outcomes, and
cost. 
(4)Accessing and utilizing health information on chronic disease for effective

(5)Supporting family caregivers and engaging them as partners with
professionals.
(6)Incentivizing providers, payers, and consumers to improve quality care and
outcomes for individuals with chronic disease.
(7)Mobilizing community resources to meet needs of persons with chronic
disease.
(8)Creating a public and private partnership that improves the outcomes for
Louisiana residents with chronic disease and helps them remain in their own homes as long
as possible.
WHEREAS, the study group requests the opportunity to delegate to the public and
private partnership the work of prioritizing, championing, and stimulating implementation
of the key recommendations and strategies; and 
WHEREAS, the study group is confident that the establishment of a public and
private partnership would focus attention, commitment, and action on improvements in
health outcomes; and   
WHEREAS, the study group is requesting authorization to meet quarterly to monitor
the progress of such improvements. 
THEREFORE, BE IT RESOLVED that the House of Representatives of the
Legislature of Louisiana does hereby extend the term of the Study Group on Chronic Care
Management in the Community and continues the group's responsibilities concerning the
following:
(1)Identifying problems related to effective care coordination.
(2)Assessing current care delivery and support services in Louisiana.
(3)Examining promising national programmatic models for coordination of care.
(4)Making recommendations for improvements that keep persons with chronic
diseases in their own homes and communities.  
BE IT FURTHER RESOLVED that the composition of the study group shall remain
unchanged.
BE IT FURTHER RESOLVED that the members of this study group shall continue
to serve without compensation, except per diem or expense reimbursement to which they
may be individually entitled as members of their constituent organizations.  ENROLLEDHR NO. 140

BE IT FURTHER RESOLVED that the study group may continue under its prior
organization, adopted rules of procedure, work schedule, and elected chairman and vice
chairman, and is authorized to adopt any additional measure that it deems necessary for the
timely performance of its duties.
BE IT FURTHER RESOLVED that all departments, boards, agencies, officers, and
institutions of the state and all subdivisions thereof shall continue to cooperate with the study
group in providing information to assist the study group in carrying out its mission. 
BE IT FURTHER RESOLVED that the authority of the study group shall expire on
June 30, 2013.
BE IT FURTHER RESOLVED that the study group shall issue a finalized,
comprehensive written report of its findings to the House of Representatives of the
Legislature of Louisiana on or before June 30, 2013.
BE IT FURTHER RESOLVED that a copy of this Resolution shall be transmitted
to the executive director of the Louisiana Public Health Institute; the executive director of
the Louisiana Health Care Quality Forum; the executive director of the Pennington
Biomedical Research Center; the chancellor of the LSU Health Sciences Center; the
secretary of the Department of Health and Hospitals; the assistant secretary of the office of
aging and adult services of the Department of Health and Hospitals; the assistant secretary
of the office of public health of the Department of Health and Hospitals; the director of
AARP Louisiana; the president and chief executive officer of the Louisiana Hospital
Association; the chief executive officer of the HomeCare Association of Louisiana; the
director of the Coalition for Choice to Advance Long Term Care Options in Louisiana; the
executive vice president of the Louisiana State Medical Society; the president of the Public
Affairs Research Council; the chief financial officer of the Rapides Foundation; the chair
executive officer of the Louisiana Health Care Commission; the director of the UNO
Transportation Institute; the executive director of the Louisiana State Board of Nursing; and
the president of the Advocacy Center.
