BE IT ENACTED BY THE GENERAL ASSEMBLY OF MARYLAND, 13 That the Laws of Maryland read as follows: 14 Article – Education 15 15–134. 16 (A) IN THIS SECTION , “CHRONIC HEALTH CONDITION ” MEANS A HEALTH 17 CONDITION OR DISEASE THAT IS PERSISTENT OR LON G–LASTING IN ITS EFFEC TS OR 18 COMES WITH TIME AND REQUIRE S ONGOING MEDICAL ATTE NTION. 19 (B) EACH PUBLIC INSTITUTION OF HIGHE R EDUCATION IN THE STATE 20 SHALL DESIGNATE A CASE MANAGER FOR STUDENTS WITH CHRONI C HEALTH 21 CONDITIONS. 22 (C) A CASE MANAGER SHALL : 23 2 HOUSE BILL 1098 (1) COORDINATE SERVICES A MONG DIFFERENT DEPAR TMENTS IN AN 1 INSTITUTION, INCLUDING: 2 (I) ACADEMIC COUNSELING ; 3 (II) DISABILITY SERVICES ; 4 (III) HOUSING AND DINING SERVICES; 5 (IV) HEALTH AND MENTAL HEALTH SERVICES; 6 (V) STUDENT ENGAGEMENT SE RVICES; 7 (VI) JOB TRAINING; AND 8 (VII) ANY OTHER RELEVANT SERVICES; 9 (2) PROVIDE INFORMATION TO STUDENTS REGARDIN G THE 10 AVAILABILITY OF AND ACCESS TO NECESSARY SUPPORTS AND SERVICES FOR 11 STUDENTS WITH CHRONI C HEALTH CONDITIONS; AND 12 (3) COLLECT DATA AND INFO RMATION ON PROGRAMS , SERVICES, 13 AND OUTCOMES, INCLUDING: 14 (I) THE NUMBER OF STUDENT S UTILIZING THE PROG RAMS AND 15 SERVICES; 16 (II) THE QUALITY AND EFFEC TIVENESS OF THE SERVICES 17 PROVIDED; 18 (III) THE SATISFACTION OF T HE STUDENTS UTILIZIN G THE 19 PROGRAMS AND SERVICES, INCLUDING T HE CASE MANAGEMENT S ERVICES; 20 (IV) HEALTH SURVEYS ; 21 (V) NEEDS ASSESSMENTS ; 22 (VI) STUDENT RECOMMENDATIO NS; 23 (VII) STUDENT ACADEMIC ACHIEVEMENT , INCLUDING 24 COMPLETION OF CREDIT S, GRADUATION RATES, NONCOMPLETION OF CRE DITS, 25 MEDICAL WITHDRAWALS , AND LEAVES OF ABSENCE; AND 26 HOUSE BILL 1098 3 (VIII) ANY OTHER DATA OR INF ORMATION DETERMINED 1 NECESSARY BY THE CAS E MANAGER OR INSTITUTION. 2 SECTION 2.
