66 1.6 Section 1. [145.904] GRANT PROGRAM; YOUTH SPORTS PHYSICALS BY​

88 1.8 Subdivision 1.Definitions.(a) For purposes of this section, the following terms have​
99 1.9the meanings given.​
1010 1.10 (b) "Commissioner" means the commissioner of health.​
1111 1.11 (c) "Mobile health evaluation and screening clinic" or "mobile clinic" means a clinic​
1212 1.12transported in a vehicle-mounted unit that is either motorized or trailered and readily movable​
1313 1.13with disassembling, and in which one or more health professionals are transported to provide​
1414 1.14evaluation and screening services in more than one geographic location.​
1515 1.15 (d) "School" has the meaning given in section 120A.22, subdivision 4, but does not​
1616 1.16include a home school.​
1717 1.17 Subd. 2.Grant program established.The commissioner must establish and administer​
1818 1.18a grant program to provide youth sports physicals at schools in Minnesota by mobile health​
1919 1.19evaluation and screening clinics. To be eligible for a grant under this section, an entity must​
2020 1.20demonstrate that the entity has matching funds from nonstate sources equal to at least 25​
2121 1.21percent of the grant amount sought. An entity seeking a grant must apply to the commissioner​
2222 1.22in a form and manner specified by the commissioner.​

3232 The bill was read for the first time and referred to the Committee on Health Finance and Policy​ 2.1 Subd. 3.Scheduling of visits; staffing.An entity receiving a grant under this section​
3333 2.2must engage in outreach with schools in Minnesota and schedule mobile clinic visits with​
3434 2.3interested schools to provide youth sports physicals to students enrolled in the school. Before​
3535 2.4a mobile clinic's visit to a school, the entity receiving the grant must provide the school with​
3636 2.5information to make available to families on the importance of preventive health care for​
3737 2.6students participating in youth sports. A mobile clinic must be staffed with at least one​
3838 2.7physician licensed under chapter 147, physician assistant licensed under chapter 147A, or​
3939 2.8advanced practice registered nurse as defined in section 148.171, subdivision 3, and may​
4040 2.9also be staffed with other health professionals.​
4141 2.10 Subd. 4.Required elements of a youth sports physical; referrals.(a) A youth sports​
4242 2.11physical provided by health professionals at a mobile clinic under this section must at least​
4343 2.12include a cardiac screening, respiratory assessment, blood pressure evaluation,​
4444 2.13musculoskeletal examination, family medical history review, and mental health screening.​
4545 2.14The mental health screening must at least include a stress and anxiety evaluation, a depression​
4646 2.15and emotional well-being assessment, an evaluation of sleep and fatigue, a substance use​
4747 2.16disorder screening, an eating disorder screening, and a provision of resources and referrals​
4848 2.17if needed.​
4949 2.18 (b) A health professional who conducts youth sports physicals under this section must​
5050 2.19refer or advise a student to seek additional evaluation or services if indicated based on the​
5151 2.20screenings and assessments conducted. A health professional who identifies concerns​
5252 2.21regarding a student's mental health must refer the student for confidential follow-up with​
5353 2.22the student's school counselor or mental health professional, or advise a student to see a​
5454 2.23local mental health professional.​
5555 2.24 (c) An entity receiving a grant must establish a policy regarding parental consent to​
5656 2.25youth sports physicals that complies with applicable federal and state laws. Health​
5757 2.26professionals conducting youth sports physicals at the entity's mobile clinic must comply​
5858 2.27with the entity's policy.​
5959 2.28 Subd. 5.Uninsured students.A mobile clinic providing youth sports physicals under​
6060 2.29this section must provide youth sports physicals free of charge to students who are uninsured.​
6161 2.30 Subd. 6.Data collection and reporting.A mobile clinic must maintain and share with​
6262 2.31the commissioner, in a form and manner specified by the commissioner, data on numbers​
6363 2.32of youth sports physicals provided, schools in which youth sports physicals are provided,​
6464 2.33numbers of referrals made for follow-up services and the reasons for referrals, and other​
6565 2​Section 1.​

6868 3.3 Sec. 2. APPROPRIATIONS.​
6969 3.4 $1,000,000 in fiscal year 2026 and $600,000 in fiscal year 2027 are appropriated from​
7070 3.5the general fund to the commissioner of health for the grant program for youth sports​
7171 3.6physicals by mobile clinics under Minnesota Statutes, section 145.904.​
