11 | 11 | | 1.11 Subd. 2.Definitions.(a) For purposes of this section, the following terms have the |

13 | 13 | | 1.13 (b) "Community-based organization" means a public or private nonprofit organization |
---|
14 | 14 | | 1.14of demonstrated effectiveness that is representative of a community or significant segments |
---|
15 | 15 | | 1.15of a community and provides services that address the social drivers of health, education, |
---|
16 | 16 | | 1.16or related services to individuals in the community. |
---|
17 | 17 | | 1.17 (c) "Community care hub" means a nonprofit organization that provides a centralized |
---|
18 | 18 | | 1.18administrative and operational interface between health care institutions and a network of |
---|
19 | 19 | | 1.19community-based organizations that provide health promotion and social care services. |
---|
20 | 20 | | 1.20 (d) "Health-related social needs" means the individual-level, adverse social conditions |
---|
21 | 21 | | 1.21that can negatively impact a person's health or health care, such as poor health literacy, food |
---|
22 | 22 | | 1.22insecurity, housing instability, and lack of access to transportation. |

35 | 35 | | 2.5 (1) be recognized as a selected community care hub by the federal Administration for |

36 | 36 | | 2.6Community Living and the Centers for Disease Control and Prevention; |

37 | 37 | | 2.7 (2) hold contracts with health plans within Minnesota that allow the applicant to provide |
---|
38 | 38 | | 2.8social care services to a plan's covered member population; and |
---|
39 | 39 | | 2.9 (3) demonstrate active engagement in providing, coordinating, and aiding health care |
---|
40 | 40 | | 2.10and social care services at the community level. |
---|
41 | 41 | | 2.11 Subd. 4.Eligible uses.The grantee must use awarded funding to: |

42 | 42 | | 2.12 (1) engage and organize community-based organizations to deliver social care services; |
---|
43 | 43 | | 2.13 (2) expand the reach and scope of social care services; |
---|
44 | 44 | | 2.14 (3) centralize administrative functions and operational infrastructure of community care |
---|
45 | 45 | | 2.15hubs related to: |
---|
46 | 46 | | 2.16 (i) contracting with health care organizations; |
---|
47 | 47 | | 2.17 (ii) payment operations; |
---|
48 | 48 | | 2.18 (iii) management of referrals; |
---|
49 | 49 | | 2.19 (iv) service delivery fidelity and compliance; |
---|
50 | 50 | | 2.20 (v) quality improvement; |
---|
51 | 51 | | 2.21 (vi) technology; |
---|
52 | 52 | | 2.22 (vii) information security; and |
---|
53 | 53 | | 2.23 (viii) data collection, data analysis, and reporting; and |

54 | 54 | | 2.24 (4) create sustainable financial pathways for services that address health-related social |
---|
55 | 55 | | 2.25needs throughout the state of Minnesota. |

56 | 56 | | 2.26 Subd. 5.Grantee report.The grantee must report community care hub initiative |

57 | 57 | | 2.27outcomes as determined by the commissioner of health to the commissioner on the forms |
---|
58 | 58 | | 2.28and according to the timelines established by the commissioner. |

64 | 64 | | 3.5 Sec. 2. APPROPRIATION; COMMUNITY CARE HUB GRANT. |

65 | 65 | | 3.6 (a) $8,900,000 in fiscal year 2026 is appropriated from the general fund to the |
---|
66 | 66 | | 3.7commissioner of health for the community care hub grant. This is a onetime appropriation |
---|
67 | 67 | | 3.8and is available until June 30, 2030. |
---|
68 | 68 | | 3.9 (b) $1,000,000 in fiscal year 2026 is appropriated from the general fund to the |
---|
69 | 69 | | 3.10commissioner of health to conduct an evaluation of the community care hub initiative |
---|
70 | 70 | | 3.11implemented by the recipient of the community care hub grant and to provide technical |
---|
71 | 71 | | 3.12assistance and other support to the grantee. This is a onetime appropriation and is available |
---|
72 | 72 | | 3.13until June 30, 2030. |
