77 1.7 Subdivision 1.Establishment.The commissioner of health shall establish a single grant​
88 1.8to expand and strengthen the community care hub model in Minnesota by organizing and​
99 1.9supporting a network of health and social care service providers to address health-related​
1010 1.10social needs.​
1111 1.11 Subd. 2.Definitions.(a) For purposes of this section, the following terms have the​
1212 1.12meanings given.​
1313 1.13 (b) "Community-based organization" means a public or private nonprofit organization​
1414 1.14of demonstrated effectiveness that is representative of a community or significant segments​
1515 1.15of a community and provides services that address the social drivers of health, education,​
1616 1.16or related services to individuals in the community.​
1717 1.17 (c) "Community care hub" means a nonprofit organization that provides a centralized​
1818 1.18administrative and operational interface between health care institutions and a network of​
1919 1.19community-based organizations that provide health promotion and social care services.​
2020 1.20 (d) "Health-related social needs" means the individual-level, adverse social conditions​
2121 1.21that can negatively impact a person's health or health care, such as poor health literacy, food​
2222 1.22insecurity, housing instability, and lack of access to transportation.​

3131 Referred to Health and Human Services​ 2.1 (e) "Social care services" means culturally informed services to address health-related​
3232 2.2social needs and community-informed health promotion programs.​
3333 2.3 Subd. 3.Eligible applicants.To be eligible for the single grant available under this​
3434 2.4section, a grant applicant must:​
3535 2.5 (1) be recognized as a selected community care hub by the federal Administration for​
3636 2.6Community Living and the Centers for Disease Control and Prevention;​
3737 2.7 (2) hold contracts with health plans within Minnesota that allow the applicant to provide​
3838 2.8social care services to a plan's covered member population; and​
3939 2.9 (3) demonstrate active engagement in providing, coordinating, and aiding health care​
4040 2.10and social care services at the community level.​
4141 2.11 Subd. 4.Eligible uses.The grantee must use awarded funding to:​
4242 2.12 (1) engage and organize community-based organizations to deliver social care services;​
4343 2.13 (2) expand the reach and scope of social care services;​
4444 2.14 (3) centralize administrative functions and operational infrastructure of community care​
4545 2.15hubs related to:​
4646 2.16 (i) contracting with health care organizations;​
4747 2.17 (ii) payment operations;​
4848 2.18 (iii) management of referrals;​
4949 2.19 (iv) service delivery fidelity and compliance;​
5050 2.20 (v) quality improvement;​
5151 2.21 (vi) technology;​
5252 2.22 (vii) information security; and​
5353 2.23 (viii) data collection, data analysis, and reporting; and​
5454 2.24 (4) create sustainable financial pathways for services that address health-related social​
5555 2.25needs throughout the state of Minnesota.​
5656 2.26 Subd. 5.Grantee report.The grantee must report community care hub initiative​
5757 2.27outcomes as determined by the commissioner of health to the commissioner on the forms​
5858 2.28and according to the timelines established by the commissioner.​

6161 3.2the community care hub initiative implemented by the grantee using measures to assess​
6262 3.3cost savings, impact, and health impact outcomes.​
6363 3.4 EFFECTIVE DATE.This section is effective July 1, 2025.​
6464 3.5 Sec. 2. APPROPRIATION; COMMUNITY CARE HUB GRANT.​
6565 3.6 (a) $8,900,000 in fiscal year 2026 is appropriated from the general fund to the​
6666 3.7commissioner of health for the community care hub grant. This is a onetime appropriation​
6767 3.8and is available until June 30, 2030.​
6868 3.9 (b) $1,000,000 in fiscal year 2026 is appropriated from the general fund to the​
6969 3.10commissioner of health to conduct an evaluation of the community care hub initiative​
7070 3.11implemented by the recipient of the community care hub grant and to provide technical​
7171 3.12assistance and other support to the grantee. This is a onetime appropriation and is available​
7272 3.13until June 30, 2030.​
