| 18 | 17 | | SECTION 1. This act shall be known as "The University Vaccination Freedom Act." 5 |

| 19 | 18 | | SECTION 2. G.S. 130A-154(b) reads as rewritten: 6 |

| 25 | 24 | | MMR and the last DTP and polio; the name and address of the physician or local health 12 |

| 26 | 25 | | department administering the required immunization; and other relevant information required by 13 |
