Requires that space be provided on applications for a civil service examination, claims for unemployment benefits, applications for the practice of a profession or occupation, state income tax returns, and applications to register a motor vehicle, for the applicant to register in the "donate life registry" for organ, eye and tissue donation established pursuant to section 4310 of the public health law.

Establishes certification procedures for the profession of nurse anesthesia; provides for the issuance of a certification to practice as a certified registered nurse anesthetist upon the filing of an application, having a license as a registered professional nurse, having the appropriate educational requirements and paying the fifty dollar initial certification fee and the triennial thirty dollar registration fee.
Enacts the "New York saves lives act" to authorize any New York state resident aged sixteen and older to register as an organ, eye and tissue donor through the New York state donate life registry; adds a New York state donate life registry option on admission applications to the state university of New York and the city university of New York.
Requires local boards of education, BOCES, charter schools and non public schools to adopt policies to promote student voter registration and pre-registration, including procedures for providing access to voter registration and pre-registration applications during the school year and assistance with filing such applications and informing students of the state requirements for voter registration and pre-registration.
