59 | 106 | C. Application for certification as a Problem Gam bling | |
60 | 107 | Treatment Counselor shall be made to the Depa rtment of Mental Health | |
61 | 108 | and Substance Abuse Services on prescribed forms authorized by the | |
62 | 109 | Department. The Board, or the Commissioner of Mental Health and | |
63 | 110 | Substance Abuse Services upon delegation by the B oard, may certify | |
64 | 111 | the Problem Gambling Treatment Co unselor for a period of two (2) | |
65 | 112 | years subject to renewal as provided in the rules promulgated by the | |
66 | 113 | Board. | |

68 | 114 | D. The Board is authorized to establish an application and | |
69 | 115 | renewal fee of no more than One Hundr ed Dollars ($100.00) to defray | |
70 | 116 | the costs incurred i n the certification process. | |

72 | 117 | E. Problem Gambling Treatment Counselors certified by the Board | |
73 | 118 | or the Commissioner shall only use the title "Problem Gambling | |
