50	50	BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
51	51	SECTION 1. AMENDATORY 26 O.S. 2021, Section 4 -112, is
52	52	amended to read as follows:
53	53	Section 4-112. A. The Secretary of the State Electi on Board
54	54	shall devise and distribute a registration application form to be
55	55	used for registering voters. Such registration application shall
56	56	contain the following information:
57	57	1. The applicant's full name and date of birth, cou nty and
58	58	place of residence, and mailing address pursuant to the p rovisions
59	59	of subsection G of this section;
60	60	2. A space or section to designate a political party recognized
61	61	by the laws of the State of Oklahom a with which the applicant
62	62	chooses to be affilia ted;

89	89	3. The Oklahoma driver license or identification card numbe r if
90	90	the applicant has been issued a current and valid driver license or
91	91	identification card by the Department of Public Safety, or if the
92	92	applicant does not have a valid Oklahoma driver lice nse or
93	93	identification card, the last four digits of the voter 's Social
94	94	Security number;
95	95	4. An oath of the eligibility of the applicant to become a
96	96	registered voter; and
97	97	5. Such other information as may be d eemed necessary by the
98	98	Secretary to identify suc h applicant and to ascertain his or her
99	99	eligibility.
100	100	B. A voter registration application shall be signed by the
101	101	applicant in writing. The applicant shall personally subscribe his
102	102	or her name to or make his or her mark on the application, and no
103	103	agent, representative or employee of the applicant may sign or mark
104	104	on the applicant's behalf. The signature or mark must be the
105	105	original, handwritten signature, autograph or mark of the applicant.
106	106	No facsimile, reproduction, typewritten or other substitute
107	107	signature, autograph or mark will be valid. Notwithstanding any law
108	108	to the contrary, the Secretary of the State Election Board shall
109	109	prescribe procedures to authorize any person incapable of personally
110	110	making a mark to complete a voter registration application with
111	111	assistance of an offic ial of any voter registration agency or motor
112	112	license agency specified in Sections 4 -109.2 and 4-109.3 of this

139	139	title. Provided, for applications submitted electronically, in lieu
140	140	of the signature requirements set forth in this subsection, the
141	141	applicant shall consent to the use of his or her driver license or
142	142	identification card signatu re as provided in Section 4-109.4 of this
143	143	title.
144	144	C. Persons who do not indicate a recognized poli tical party or
145	145	political organization on their r egistration application shal l be
146	146	designated as Independents.
147	147	D. Any person may apply in writing to the Secretary of the
148	148	State Election Board for permission to print, copy or otherwise
149	149	prepare and distribute the registration applications designed by the
150	150	Secretary of the State Electio n Board. The Secretary may revoke an y
151	151	such permission at any time.
152	152	E. All registration applications shall be distributed to the
153	153	public at no charge.
154	154	F. The Secretary also shall prescribe procedures to accept and
155	155	use the National Mail Voter Registration Form, or its successor, as
156	156	required by the National Voter Registration Act of 1993. Provided,
157	157	to be accepted as a valid voter registration application, the form
158	158	shall include the applicant's original, handwritten signature,
159	159	autograph or mark as described in subsection B of this section.
160	160	G. Applicants for voter registration or for change of voter
161	161	registration in any way shall provide a residence address and, if
162	162	different from the r esidence address, a mailing address. A

189	189	residence address shall include the street address of the residence,
190	190	including a full house number, street name or number, apartment or
191	191	suite number, if applicable, and zip code. If a street address is
192	192	not available for the residence, applicants shall provide suc h
193	193	information as the Secreta ry of the State Election Board deems
194	194	necessary for voter registration purposes. A post office box may
195	195	not be given as a residence address. A mailing address, which shall
196	196	include the city and zip code, may be the actual emergen cy
197	197	notification or 911 addre ss on file in the local community, a rural
198	198	route and box number, a post office box number or a street address.
199	199	H. A full or partial Social Security number or driver license
200	200	number in a voter registration record or a voter regis tration
201	201	application shall no t be considered a public record and s hall be
202	202	kept confidential by the State Election Board and each county
203	203	election board.
204	204	I. The Secretary of State shall establish standards to verify
205	205	the authenticity of the information requir ed in paragraph 3 of
206	206	subsection A of this sect ion for every registered voter in the
207	207	state.
208	208	SECTION 2. This act shall become effective November 1, 2022.
