5050 BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
5151 SECTION 1. AMENDATORY 26 O.S. 2021, Section 4 -112, is
5252 amended to read as follows:
5353 Section 4-112. A. The Secretary of the State Electi on Board
5454 shall devise and distribute a registration application form to be
5555 used for registering voters. Such registration application shall
5656 contain the following information:
5757 1. The applicant's full name and date of birth, cou nty and
5858 place of residence, and mailing address pursuant to the p rovisions
5959 of subsection G of this section;
6060 2. A space or section to designate a political party recognized
6161 by the laws of the State of Oklahom a with which the applicant
6262 chooses to be affilia ted;

8989 3. The Oklahoma driver license or identification card numbe r if
9090 the applicant has been issued a current and valid driver license or
9191 identification card by the Department of Public Safety, or if the
9292 applicant does not have a valid Oklahoma driver lice nse or
9393 identification card, the last four digits of the voter 's Social
9494 Security number;
9595 4. An oath of the eligibility of the applicant to become a
9696 registered voter; and
9797 5. Such other information as may be d eemed necessary by the
9898 Secretary to identify suc h applicant and to ascertain his or her
9999 eligibility.
100100 B. A voter registration application shall be signed by the
101101 applicant in writing. The applicant shall personally subscribe his
102102 or her name to or make his or her mark on the application, and no
103103 agent, representative or employee of the applicant may sign or mark
104104 on the applicant's behalf. The signature or mark must be the
105105 original, handwritten signature, autograph or mark of the applicant.
106106 No facsimile, reproduction, typewritten or other substitute
107107 signature, autograph or mark will be valid. Notwithstanding any law
108108 to the contrary, the Secretary of the State Election Board shall
109109 prescribe procedures to authorize any person incapable of personally
110110 making a mark to complete a voter registration application with
111111 assistance of an offic ial of any voter registration agency or motor
112112 license agency specified in Sections 4 -109.2 and 4-109.3 of this

139139 title. Provided, for applications submitted electronically, in lieu
140140 of the signature requirements set forth in this subsection, the
141141 applicant shall consent to the use of his or her driver license or
142142 identification card signatu re as provided in Section 4-109.4 of this
143143 title.
144144 C. Persons who do not indicate a recognized poli tical party or
145145 political organization on their r egistration application shal l be
146146 designated as Independents.
147147 D. Any person may apply in writing to the Secretary of the
148148 State Election Board for permission to print, copy or otherwise
149149 prepare and distribute the registration applications designed by the
150150 Secretary of the State Electio n Board. The Secretary may revoke an y
151151 such permission at any time.
152152 E. All registration applications shall be distributed to the
153153 public at no charge.
154154 F. The Secretary also shall prescribe procedures to accept and
155155 use the National Mail Voter Registration Form, or its successor, as
156156 required by the National Voter Registration Act of 1993. Provided,
157157 to be accepted as a valid voter registration application, the form
158158 shall include the applicant's original, handwritten signature,
159159 autograph or mark as described in subsection B of this section.
160160 G. Applicants for voter registration or for change of voter
161161 registration in any way shall provide a residence address and, if
162162 different from the r esidence address, a mailing address. A

189189 residence address shall include the street address of the residence,
190190 including a full house number, street name or number, apartment or
191191 suite number, if applicable, and zip code. If a street address is
192192 not available for the residence, applicants shall provide suc h
193193 information as the Secreta ry of the State Election Board deems
194194 necessary for voter registration purposes. A post office box may
195195 not be given as a residence address. A mailing address, which shall
196196 include the city and zip code, may be the actual emergen cy
197197 notification or 911 addre ss on file in the local community, a rural
198198 route and box number, a post office box number or a street address.
199199 H. A full or partial Social Security number or driver license
200200 number in a voter registration record or a voter regis tration
201201 application shall no t be considered a public record and s hall be
202202 kept confidential by the State Election Board and each county
203203 election board.
204204 I. The Secretary of State shall establish standards to verify
205205 the authenticity of the information requir ed in paragraph 3 of
206206 subsection A of this sect ion for every registered voter in the
207207 state.
208208 SECTION 2. This act shall become effective November 1, 2022.
