77	77	BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
78	78	SECTION 1. AMENDATORY 26 O.S. 2021, Section 5 -111, is
79	79	amended to read as follows:
80	80	Section 5-111. A. Forms to be used for filing Declarations of
81	81	Candidacy shall be prescribed by the Secretary of t he State Election
82	82	Board and shall contain the following information:
83	83	1. The name of the candidate as it shall a ppear on the ballot;
84	84	2. The legal name of the candidate;
85	85	3. The candidate’s place of residence and mailing address;
86	86	4. The name of the office sought;
87	87	5. The candidate’s date of birth;

139	139	6. The party affiliation of the candidate seeking political
140	140	party nomination regardless of whether the office sought is partisan
141	141	or nonpartisan;
142	142	7. The precinct and county wherein the candidate is a
143	143	registered voter;
144	144	8. An oath wherein the candidate swears or affirms that he or
145	145	she is qualified to become a candidate for the office s ought, and
146	146	that, if elected, the candidate will be qua lified to hold the
147	147	office; and
148	148	9. Any additional information which the Secre tary deems
149	149	necessary.
150	150	B. If the candidate has ever been ultimately determined by a
151	151	court of proper authority to be guilty of an offense specified in
152	152	subsection A of Section 5 -105a of this title or at the t ime of
153	153	filing the Declaration of Candidacy is name d in an outstanding
154	154	warrant for arrest for such an offense, in this or any other state,
155	155	the candidate shall provide the follo wing information on a form
156	156	prescribed by the Secretary of the State Election Boar d:
157	157	1. The name of the offense;
158	158	2. The date of conviction or issuance of the outstanding
159	159	warrant; and
160	160	3. The county and state of conviction or issuance of the
161	161	outstanding warrant.

213	213	The provisions of this subsection shall not app ly to an offense
214	214	for which the candidate has received a pardon.
215	215	C. A Declaration of Candidacy form must be signed by the
216	216	candidate, and the signature must be properly notarized by a notary
217	217	public or other person authorized by law to administer oaths.
218	218	D. In addition to the informat ion required by this section, a
219	219	candidate may include a telephone number, email address and website
220	220	address. Such additional information shall not be required of any
221	221	candidate but if provided shall be made available to the publi c.
222	222	SECTION 2. AMENDATORY 26 O.S. 2021, Section 6 -101, is
223	223	amended to read as follows:
224	224	Section 6-101. The name of any candidate for any office shall
225	225	be printed on the official ballot a s said the candidate is
226	226	designated on the declaration of candidacy; provid ed, however, that
227	227	no candidate shall have any pref ix, suffix or title placed before or
228	228	after the candidate’s name. The party affiliation of the candidate
229	229	shall be printed on the bal lot with the name of the cand idate
230	230	regardless of whether the office sought is partisan or nonpartisa n.
231	231	SECTION 3. This act shall become effective November 1, 2022.
