79	79	BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
80	80	SECTION 1. AMENDATORY 26 O.S. 2011, Section 5 -111, as
81	81	last amended by Section 1, Chapter 402, O.S.L. 2013 (26 O.S. Supp.
82	82	2020, Section 5-111), is amended to read as follows:
83	83	Section 5-111. A. Forms to be used for filing Declarations of
84	84	Candidacy shall be prescribed by the Secre tary of the State Elec tion
85	85	Board and shall contain the following information:
86	86	1. The name of the candidate as it shall a ppear on the ballot;
87	87	2. The legal name of the candid ate;
88	88	3. The candidate’s place of residence and maili ng address;
89	89	4. The name of the office sought;

142	142	6. The party affiliation of the candidate seeking political
143	143	party nomination regardless of whether the office s ought is partisan
144	144	or nonpartisan;
145	145	7. The precinct and county wherein the candidate is a
146	146	registered voter;
147	147	8. An oath wherein the candidate swears or affirms that he or
148	148	she is qualified to become a candidate for the office sought, and
149	149	that, if elected, the candida te will be qualified to hold the
150	150	office; and
151	151	9. Any additional information which the Secretary deems
152	152	necessary.
153	153	B. If the candidate has ever been ultimately determined by a
154	154	court of proper authority to be guilty of an offense specified in
155	155	subsection A of Section 5-105a of this title or at the time of
156	156	filing the Declaration of Candidac y is named in an outst anding
157	157	warrant for arrest for such an offense, in this or any other state,
158	158	the candidate shall provide the following information on a form
159	159	prescribed by the Secretary of the State Election Board:
160	160	1. The name of the offense;
161	161	2. The date of conviction or i ssuance of the outstanding
162	162	warrant; and
163	163	3. The county and state of conviction or issuance of the
164	164	outstanding warrant.

216	216	The provisions of this subsection shall not apply to an offense
217	217	for which the candidat e has received a pardon.
218	218	C. A Declaration of Candi dacy form must be signed by the
219	219	candidate, and the signature must be properly notarized by a notary
220	220	public or other person authorized by law to administ er oaths.
221	221	D. In addition to the information required by this section, a
222	222	candidate may include a telepho ne number, email address and website
223	223	address. Such additional information shall not be required of any
224	224	candidate but if provided shall be made availabl e to the public.
225	225	SECTION 2. AMENDATORY 26 O.S. 2011, Sec tion 6-101, as
226	226	amended by Section 2, Chapter 96, O.S.L. 2013 (26 O.S. Supp. 2020,
227	227	Section 6-101), is amended to read as follows:
228	228	Section 6-101. The name of any candidate for any o ffice shall
229	229	be printed on the official ballot a s said the candidate is
230	230	designated on the declarati on of candidacy; provided, however, that
231	231	no candidate shall have any prefix, suffix o r title placed before or
232	232	after the candidate’s name. The party affiliation of the candidate
233	233	shall be printed on the bal lot with the name of the cand idate
234	234	regardless of whether the office sought is partisan or nonpartisa n.
235	235	SECTION 3. This act shall become effective November 1, 2021.
