7979 BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
8080 SECTION 1. AMENDATORY 26 O.S. 2011, Section 5 -111, as
8181 last amended by Section 1, Chapter 402, O.S.L. 2013 (26 O.S. Supp.
8282 2020, Section 5-111), is amended to read as follows:
8383 Section 5-111. A. Forms to be used for filing Declarations of
8484 Candidacy shall be prescribed by the Secre tary of the State Elec tion
8585 Board and shall contain the following information:
8686 1. The name of the candidate as it shall a ppear on the ballot;
8787 2. The legal name of the candid ate;
8888 3. The candidate’s place of residence and maili ng address;
8989 4. The name of the office sought;

141141 5. The candidate’s date of birth;
142142 6. The party affiliation of the candidate seeking political
143143 party nomination regardless of whether the office s ought is partisan
144144 or nonpartisan;
145145 7. The precinct and county wherein the candidate is a
146146 registered voter;
147147 8. An oath wherein the candidate swears or affirms that he or
148148 she is qualified to become a candidate for the office sought, and
149149 that, if elected, the candida te will be qualified to hold the
150150 office; and
151151 9. Any additional information which the Secretary deems
152152 necessary.
153153 B. If the candidate has ever been ultimately determined by a
154154 court of proper authority to be guilty of an offense specified in
155155 subsection A of Section 5-105a of this title or at the time of
156156 filing the Declaration of Candidac y is named in an outst anding
157157 warrant for arrest for such an offense, in this or any other state,
158158 the candidate shall provide the following information on a form
159159 prescribed by the Secretary of the State Election Board:
160160 1. The name of the offense;
161161 2. The date of conviction or i ssuance of the outstanding
162162 warrant; and
163163 3. The county and state of conviction or issuance of the
164164 outstanding warrant.

216216 The provisions of this subsection shall not apply to an offense
217217 for which the candidat e has received a pardon.
218218 C. A Declaration of Candi dacy form must be signed by the
219219 candidate, and the signature must be properly notarized by a notary
220220 public or other person authorized by law to administ er oaths.
221221 D. In addition to the information required by this section, a
222222 candidate may include a telepho ne number, email address and website
223223 address. Such additional information shall not be required of any
224224 candidate but if provided shall be made availabl e to the public.
225225 SECTION 2. AMENDATORY 26 O.S. 2011, Sec tion 6-101, as
226226 amended by Section 2, Chapter 96, O.S.L. 2013 (26 O.S. Supp. 2020,
227227 Section 6-101), is amended to read as follows:
228228 Section 6-101. The name of any candidate for any o ffice shall
229229 be printed on the official ballot a s said the candidate is
230230 designated on the declarati on of candidacy; provided, however, that
231231 no candidate shall have any prefix, suffix o r title placed before or
232232 after the candidate’s name. The party affiliation of the candidate
233233 shall be printed on the bal lot with the name of the cand idate
234234 regardless of whether the office sought is partisan or nonpartisa n.
235235 SECTION 3. This act shall become effective November 1, 2021.
