BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA: SECTION 1. NEW LAW A new section of law to be codified in the Oklahoma Statutes as Section 2006 of Title 25, unless there is created a duplication in num bering, reads as follows: A. This section shall be known and may be cited as the “Parental Rights Immunization Act ”. B. Each health care provider who administers a vaccine set forth in the Vaccine Injury Table, as established by the United States Department of Health and Human Services, sha ll obtain informed consent from the legal representa tive or other adult authorized by law to consent on behalf of a minor to whom such provider intends to administer such vaccine. C. Providing a copy of the Vaccine Information Statement, the source of which is clearly identified, and m aking available for review the Centers for Disease C ontrol and Prevention ’s “Epidemiology and Prevention of Vaccine -Preventable Diseases: Vaccine Excipient and Media Summary ”, the Centers for Disease Control and Preventi on’s Vaccine Injury Table or the Ce nters for Disease Control and Prevention’s contraindications and precautions or any combination of these three resourc es to the legal representative or other adult authorized by law to consent on behalf of a minor shall constitute a minimum standard of rel evant information.
