4855 BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
4956 SECTION 1. AMENDATORY 26 O.S. 2021, Section 4 -112, as
5057 amended by Section 18, Chapter 282, O.S.L. 2022 (26 O.S. Supp. 2022,
5158 Section 4-112), is amended to read as follows:
5259 Section 4-112. A. The Secretary of the State Election Board
5360 shall devise and distribute a registrati on application form to be
5461 used for registering voters. Such registration applica tion shall
5562 contain the following information:
5663 1. The applicant's full name and date of birth, county and
5764 place of residence, and mailing address pursuant to the provisions
5865 of subsection G of this section;

8593 2. A space or section to designate a political pa rty recognized
8694 by the laws of the State of Oklaho ma with which the applicant
8795 chooses to be affiliated;
88-3. a. The Oklahoma driver license or identification card
89-number if the applicant has been issued a current and
90-valid driver license or identification card by Service
91-Oklahoma, or if,
92-b. If the applicant does not have a valid has not been
93-issued a current and valid Oklahoma driver license or
94-identification card, the last four di gits of the
95-voter's applicant's Social Security number, or
96-c. If the applicant attests that he or she has not been
97-issued a current and valid driver license or a Social
98-Security number, the applicant shall provi de with the
99-application an original or readab le copy of a current
100-utility bill, bank statement, governmen t check,
101-paycheck, or other government document , excluding a
102-voter identification card , that shows the name and
103-address of the applicant ;
96+3. The Oklahoma driver license or identification card number if

98+identification card by Service Oklahoma, or if the applicant does
99+not have a valid Oklahoma driver license or identification card, the

107107 Secretary to identify such applicant and to ascerta in his or her

109+B. A voter registration application shall be signed by the
110+applicant in writing. The applicant shall personally subscribe his
111+or her name to or make hi s or her mark on the application, and no
112+agent, representative or employee o f the applicant may sign or mark
113+on the applicant's behalf. The signature or mark must be the
114+original, handwritten signature, autograph or mark of the applicant .
115+No facsimile, reproduction, typewritten or other substitute
116+signature, autograph or mark wi ll be valid. Notwithstanding any law

135-B. A voter registration application shall be signed by the
136-applicant in writing. The applicant shall personally subscribe his
137-or her name to or make his or her mark on the application, and no
138-agent, representative or employee of the applicant may sign or mark
139-on the applicant's behalf. The signature or mark must be the
140-original, handwritten signature, autograph or mark of the applicant .
141-No facsimile, reproduction, typewritten or other substitute
142-signature, autograph or mark will be valid . Notwithstanding any law
143144 to the contrary, the Secretary of the State Election Board shall
144145 prescribe procedures to authorize any person incapable of personally
145146 making a mark to complete a voter registration application with
146147 assistance of an off icial of any voter registrat ion agency or
147148 licensed operator specified in Sections 4-109.2 and 4-109.3 of this
148149 title. Provided, for applic ations submitted electronically, in lieu
149150 of the signature requirements set forth in this subsection, the
150151 applicant shall consent to the use of his or her driver license or
151152 identification card signature as provided in Section 4-109.4 of this
152153 title.
153154 C. Persons who do not indicate a recognize d political party or
154155 political organization on their registration application shall be
155156 designated as Independents.
156157 D. Any person may apply in writing to the Secretary of the
157158 State Election Board for permission to print, copy or otherwise
158159 prepare and distribute the registration applications designed b y the
160+Secretary of the State Election Board. The Secretary may revoke any
161+such permission at any time.
162+E. All registration applicat ions shall be distributed to the
163+public at no charge.
164+F. The Secretary also shall prescribe procedures to accept and
165+use the National Mail Voter Registration F orm, or its successor, as
166+required by the National V oter Registration Act of 1993. Provided,
167+to be accepted as a valid voter registration application, the form

185-Secretary of the State Election Boar d. The Secretary may revoke any

187-E. All registration applicat ions shall be distributed to the
188-public at no charge.
189-F. The Secretary also shall prescribe procedures to accept and
190-use the National Mail Voter Registration Form , or its successor, as
191-required by the National V oter Registration Act of 1993. Provided,
192-to be accepted as a valid voter registration application, the form
193195 shall include the applicant's original, handwritten signat ure,
194196 autograph or mark as described i n subsection B of this section.
195197 G. Applicants for voter registration or for change of voter
196198 registration in any way shall provide a resid ence address and, if
197199 different from the residence address, a mailing address. A
198200 residence address shall include the s treet address of the residence,
199201 including a full hou se number, street name or number, apartment or
200-suite number, if applicable, and zip ZIP code. If a street address
201-is not available for the residence, applicants shall provide such
202+suite number, if applicable, and zip co de. If a street address is
203+not available for the residence, applicants shall pro vide such
202204 information as the Secretar y of the State Election Board deems
203205 necessary for voter registration purposes. A post office box may
204206 not be given as a residence address . A mailing address, which shall
205-include the city and zip ZIP code, may be the actual emergency
207+include the city and zip code, may be the actual emergency
206208 notification or 911 addres s on file in the local community, a rural
207209 route and box number, a post office box number or a st reet address.
210+H. A full or partial Social Security number or driver license
211+number in a voter registration record or a vot er registration
212+application shall not be considered a public record and shall be
213+kept confidential by the State Election Board and ea ch county
214+election board.
215+SECTION 2. This act shall become effectiv e November 1, 2023.

234-H. 1. A full or partial Social Security number or driver
235-license number in a voter registration record or a voter
236-registration application shall not be considered a public record and
237-shall be kept confidential by the State Election Board and ea ch

239-2. A current utility bill, bank statement, government check,
240-paycheck, or other government document provided with a voter
241-registration application shall not be co nsidered a public record and
242-shall be kept confidential. The secretary of the county election
243-board shall retain the item in a secure manner for ninety ( 90) days,
244-after which time the secretary shall destroy or shred the item.
245-SECTION 2. This act shall become effectiv e November 1, 2023.
