66	66	state governmental entities organized within the
67	67	executive branch of state government from requiring
68	68	certain evidence related to surgical procedures;

75	75	BE IT ENACTED BY THE PEOPLE OF T HE STATE OF OKLAHOMA:
76	76	SECTION 1. NEW LAW A new section of law to b e codified
77	77	in the Oklahoma Statutes as Section 9501 of Title 74, unless there
78	78	is created a duplication in numbering, reads as follows:
79	79	No agency, board, commission, department, instrumentality , or
80	80	other entity organized within the executive branch of state
81	81	government shall require, if applicable, on any form or other
82	82	document used by the s tate governmental entity as part of a process
83	83	for changing gender identification on a driver licen se or any permit
84	84	or other form of identification used by such entity an affidavit,
85	85	attesting to the applicant 's gender identity. The applicant 's
86	86	affidavit shall be accepted as conclusive proof of gender change if
87	87	it contains substantially the following language: "I, [name of
