7575 BE IT ENACTED BY THE PEOPLE OF THE STATE O F OKLAHOMA:
7676 SECTION 1. AMENDATORY 26 O.S. 2021, Section 4-112, as
7777 amended by Section 18, Chapter 282, O.S.L. 2022 (26 O.S. Supp. 2023,
7878 Section 4-112), is amended to read as follows:
7979 Section 4-112. A. The Secretary of the State Election Board
8080 shall devise and distribute a registrati on application form to b e
8181 used for registering voters. Such registration application shall
8282 contain the following information:
8383 1. The applicant’s full name and date of birth, county and
8484 place of residence, and mailing address pursuant to the provisions
8585 of subsection G of this s ection;

137137 2. A space or section to designate a political party recognized
138138 by the laws of the State of Oklahoma with which the applicant
139139 chooses to be affiliated;
140140 3. The Oklahoma driver license or identification card number if
141141 the applicant has been issued a current and valid driver license or
142142 identification card by Service Oklahoma, or if the applicant does
143143 not have a valid Oklahoma driver license or identification card, the
144144 last four digits of the voter’s Social Security number;
145145 4. An oath of the eligibility of the applicant to become a
146146 registered voter; and
147147 5. Such other information as may be d eemed necessary by the
148148 Secretary to identify such applicant and to ascertain his or he r
149149 eligibility.
150150 B. A voter registration application shall be signed by the
151151 applicant in writing. The applicant shall personally subscribe his
152152 or her name to or make his or her mark on the application, and no
153153 agent, representative or employee of the applic ant may sign or mark
154154 on the applicant’s behalf. The signature or mark must be the
155155 original, handwritten signature, autograph or mark of the applicant .
156156 No facsimile, reproduction, typewritten or other substitute
157157 signature, autograph or mark will be valid . Notwithstanding any law
158158 to the contrary, the Secretary of the State Election Board shall
159159 prescribe procedures to authorize any person incapable of personally
160160 making a mark to complete a voter registration application with

212212 assistance of an official of any voter registration agency or
213213 licensed operator specified in Sections 4-109.2 and 4-109.3 of this
214214 title. Provided, for applicat ions submitted electronically, in lieu
215215 of the signature requirements set forth in this subsection, the
216216 applicant shall consent to the use of his or her driver license or
217217 identification card signature as provided in Section 4-109.4 of this
218218 title.
219219 C. Persons who do not indicate a recognize d political party or
220220 political organization on their registration application shall be
221221 designated as Independents.
222222 D. Any person may apply in writing to the Secretary of the
223223 State Election Board for permission to print, co py or otherwise
224224 prepare and distribute the registration applications designed by the
225225 Secretary of the State Election Boar d. The Secretary may revoke any
226226 such permission at any time.
227227 E. All registration applicat ions shall be distributed to the
228228 public at no charge.
229229 F. The Secretary also shall prescribe procedures to accept and
230230 use the National Mail Voter Registration Form , or its successor, as
231231 required by the National Voter Registration Act of 1993. Provided,
232232 to be accepted as a valid voter registration a pplication, the form
233233 shall include the applicant’s original, handwritten signature,
234234 autograph or mark as described in s ubsection B of this section.

286286 G. Applicants for voter registration or for change of voter
287287 registration in any way shall provide a residen ce address and, if
288288 different from the residence address, a mailing address. A
289289 residence address shall include the str eet address of the residence,
290290 including a full house number, street name or number, apartment or
291291 suite number, if applicable, and zip code . If a street address is
292292 not available for the residence, applicants shall provide such
293293 information as the Secretary of the State Election Board deems
294294 necessary for voter registration purposes. A post office box may
295295 not be given as a residence address . A mailing address, which shall
296296 include the city and zip code, may be the actual emergency
297297 notification or 911 address on file in the local community, a rural
298298 route and box number, a post office box number or a street address.
299299 The secretary of a county ele ction board may utilize global
300300 positioning system (GPS) technology to ensure a voter is assigned to
301301 the appropriate precinct.
302302 H. A full or partial Social Security number or driver license
303303 number in a voter registration record or a voter registration
304304 application shall not be considered a public record and shall be
305305 kept confidential by the State Election Board and ea ch county
306306 election board.
307307 SECTION 2. This act shall become effective November 1, 2024.
