50	50	BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
51	51	SECTION 1. AMENDATORY 34 O.S. 2021, Section 2, as
52	52	amended by Section 2, Chapter 119, O.S.L. 2 024 (34 O.S. Supp. 2024,
53	53	Section 2), is amended to read as follows:
54	54	Section 2. A. The form of initiative petition shall be
55	55	substantially as follows:
56	56	INITIATIVE PETITION
57	57	To the Honorable ______________, Governor of Oklahoma (or To the
58	58	Honorable ______________, Mayor, Chairman of County Commissioners,
59	59	or other chief executive officers, as the case may be, for the city,
60	60	county or other municipality): We, the undersigned legal vote rs of
61	61	the State of Oklahoma (and of the district of _______________,
62	62	county of _______________, or city of ________________, as the case

91	91	charter, as the case may be) shall be submitted to the legal voters
92	92	of the State of Oklahoma (or of the district of ______________,

96	96	of _________, 20__, and each for himself says: I have personally
97	97	signed this petition; I am a legal voter of the State of Oklahoma

99	99	city of ______________, as the case may be); the following five data
100	100	points shall be included on the form: the voter 's legal first name,
101	101	legal last name that substantially conforms to the name in their
102	102	precinct registry, zip code, house number, and numerical month and
103	103	day of my birth. The time for f iling this petition expires ninety
104	104	(90) days from (Secretary of State to insert assigned date when
105	105	petition is to begin signature circulation). (This for State
106	106	initiative. For county, city, or other municipality the length of
107	107	time shall be ninety (90) days.) The question we herewith submit to
108	108	our fellow voters is: Shall the following bill (or proposed
109	109	amendment to the Constitution or resolution) be approved? (Insert

142	142	section must be matched to the voter registration file.
