| 146 | 146 | | 3. Except as provided for in subsections G and H of this |
|---|
| 147 | 147 | | section, every applicant for a driver license or identification card |
|---|
| 148 | 148 | | shall provide to Service Oklahoma at the time of application a |
|---|
| 149 | 149 | | document showing proof of identity. Service Oklahoma shall |
|---|
| 150 | 150 | | promulgate rules prescribing forms of identification acceptable for |
|---|
| 151 | 151 | | an original Oklahoma drive r license. |
|---|
| 152 | 152 | | B. Every applicant for a driver license shall provide the |
|---|
| 153 | 153 | | following information: |
|---|
| 154 | 154 | | 1. Full name; |
|---|
| 155 | 155 | | 2. Date of birth; |
|---|
| 156 | 156 | | 3. Sex; |
|---|
| 157 | 157 | | 4. Address of principal residence and county of such residence |
|---|
| 158 | 158 | | which shall be referenced on the REAL ID Compliant Drive r License or |
|---|
| 159 | 159 | | Identification Card; proof of principal residency, as prescribed by |
|---|

| 227 | 227 | | 10. Whether the applicant i s an alien eligible to be considered |
|---|
| 228 | 228 | | for licensure and is not prohibited from licensure pursuant to |
|---|
| 229 | 229 | | paragraph 9 of subsection A of Section 6 -103 of this title; |

| 230 | 230 | | 11. Whether the applicant has: |

| 296 | 296 | | C. 1. In addition to the requi rements of subsections A and B |
|---|
| 297 | 297 | | of this section, every applicant for a commercial driver license who |
|---|
| 298 | 298 | | is subject to the requirements of 49 C.F.R., Part 391, and is |
|---|
| 299 | 299 | | applying for an original, renewal, or replacement license, and every |
|---|
| 300 | 300 | | person who, upon or after May 8, 2012, is currently the holder of a |
|---|
| 301 | 301 | | commercial driver license and is su bject to the requirements of 49 |
|---|
| 302 | 302 | | C.F.R., Part 391, and who does not apply for a renewal or |
|---|
| 303 | 303 | | replacement license prior to January 30, 2014, shall submit to |
|---|
| 304 | 304 | | Service Oklahoma and main tain with Service Oklahoma a current |
|---|
| 305 | 305 | | approved medical examination certificate signed by a licensed |
|---|
| 306 | 306 | | physician authorized to perform and approve medical examination |
|---|
| 307 | 307 | | certifications. Service Oklahoma shall adopt rules for maintaining |
|---|
| 308 | 308 | | medical examination certi ficates pursuant to the requirements in 49 |
|---|
| 309 | 309 | | C.F.R., Parts 383 and 384. Any com mercial driver licensee subject |
|---|
| 310 | 310 | | to the requirements of this paragraph who fails to maintain on file |
|---|
| 311 | 311 | | with Service Oklahoma a current, approved medical examination |
|---|
| 312 | 312 | | certificate shall have the driving privileges of the person |
|---|

| 375 | 375 | | applicant shall serve the period of disqualification before a |
|---|
| 376 | 376 | | commercial driver license is issued to the applicant; provided, |
|---|
| 377 | 377 | | nothing in this paragraph shall be construed to prevent the issuance |
|---|
| 378 | 378 | | of a Class D driver license to the applicant. |
|---|
| 379 | 379 | | 3. If the applicant has or is app lying for a hazardous material |
|---|
| 380 | 380 | | endorsement, the applicant shall submit to a security threat |
|---|
| 381 | 381 | | assessment performed by the Transportation Security Administration |
|---|
| 382 | 382 | | of the United States Department of Homeland Security as required by |
|---|
| 383 | 383 | | and pursuant to 49 C.F.R., Pa rt 1572, which shall be used to |
|---|
| 384 | 384 | | determine whether the applicant is eligible fo r the endorsement |
|---|
| 385 | 385 | | pursuant to federal law and regulation. |
