67	67	requiring the State Department of Health to develop
68	68	and publish certain form; providing for codification;

75	75	BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:
76	76	SECTION 1. NEW LAW A new section of law to be codified
77	77	in the Oklahoma Statutes as Section 1 -702g of Title 63, unless there
78	78	is created a duplication in numbering, reads as follows:
79	79	A. When a fetal death occurs in a hospital, or when a woman
80	80	presents to a hospital for treatment of a miscarriage, the hospital
81	81	shall, prior to discharge :
82	82	1. Provide the woman or her designee with a physical or digital
83	83	copy of the form described in subsection B of this section ; and
84	84	2. Ensure that a hospital staff member verbally communicates
85	85	that the woman or family may request the ce rtificates listed on the
86	86	form through the process described on the form.

138	138	B. The State Department of Health shall , for the purposes of
139	139	this section, develop and publish on its public website a form that:
140	140	1. Explains the process for requesting a fetal deat h
141	141	certificate under Section 1 -318 of Title 63 of the Oklahoma Statutes
142	142	and the process for requesting a certificate of birth resulting in
143	143	stillbirth under Section 1 -318.2 of Title 63 of the Oklahoma
144	144	Statutes; and
145	145	2. Provides contact information for the Division of Vital
146	146	Records of the Department .
147	147	SECTION 2. This act shall become effective November 1, 2025.
